介入治疗缺血性脑卒中患者颈动脉狭窄的疗效

The clinical effect of interventional therapy on carotid stenosis in patients with ischemic stroke

目的探讨介入治疗缺血性脑卒中患者颈动脉狭窄的临床效果。方法选取该院2015年4月至2016年4月收治的60例缺血性脑卒中且颈动脉狭窄的患者,按是否接受介入治疗分为对照组和观察组,每组30例。对照组使用内科药物保守治疗,观察组使用微创介入治疗。比较两组患者治疗后颈动脉狭窄程度、美国国立卫生研究院卒中量表(NIHSS)评分、改良Rankin量表(m RS)评分及意外事件发生率。结果治疗后,观察组患者颈动脉狭窄程度明显优于对照组,两组患者NIHSS及m RS评分均低于治疗前,且观察组患者NIHSS及m RS评分均低于对照组,差异均有统计学意义(P<0.05);观察组患者随访期间意外事件发生率低于对照组,差异有统计学意义(P<0.05)。结论介入治疗缺血性脑卒中颈动脉狭窄疗效确切,可恢复颈动脉畅通,降低颈动脉狭窄程度,提高患者生活质量,降低心脑血管意外的发生。

Objective To investigate the clinical effect of interventional therapy on carotid vascular stenosis in patients with ischemic stroke. Methods According to whether received interventional therapy,the 60 patients with ischemic stroke and cervical vascular stenosis in Hunan Provincial People＇s Hospital from April 2015 to April 2016 were divided into control group and observation group, 30 cases in each group. The control group was treated with internal medicine, the observation group was treated with minimally invasive interventional therapy. The degree of carotid stenosis, NIH stroke scale （NIHSS） scores, modified Rankin scale （mRS） scores and the incidence of accidents of the two groups were compared. Results After treatment, the degree of carotid stenosis in the observation group was significantly better than that in the control group ,the NHISS scores and mRS scores of the two groups were better than those before treatment, and the NIHSS scores and mRS scores of the observation group were lower than those of the control group, the differences were statistically significant （P〈0.05）. The incidence of accidents in the ob- servation group was lower than that in the control group,the difference was statistically significant （P〈0.05）. Conclusion Interventional therapy is effective in the treatment of ischemic stroke and cervical vascular stenosis. It can restore the smooth flow of carotid artery, reduce the degree of carotid stenosis, improve the quality of life of patients and reduce the incidence of cardiovascu- lar and cerebrovascular accidents.
