2型糖尿病伴中重度牙周炎与血脂炎症因子的相关性研究 被引量:5

摘要 目的探讨血脂、血清炎症因子与2型糖尿病(T2DM)伴中、重度牙周炎的相关性。方法选取2013年5月至2014年5月西南医科大学附属医院内分泌科收治的53例T2DM患者,分为T2DM伴中、重度牙周炎组26例(A组),单纯T2DM组(不伴中、重度牙周炎)27例(B组);另选择同期在西南医科大学附属口腔医院检查者,牙周炎组25例(C组),健康对照组25例(D组)。记录四组患者牙周情况[探诊出血(BOP)、探诊深度(PD)、附着丧失(AL)],检测各组空腹血糖(FPG),血脂[总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]及血清炎症因子[超敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)]水平。结果各组患者牙周BOP阳性率、PD、AL比较,差异均有统计学意义(P<0.05)。各组间FPG、TC、TG、LDL-C、HDL-C、hs-CRP、TNF-α比较,差异均有统计学意义(P<0.05)。FPG、TG、TC、LDL-C、hs-CRP、TNF-α均与PD、AL呈正相关(P<0.05);HDL-C与PD、AL呈负相关(P<0.05)。结论中、重度牙周炎加重了糖尿病的发生,血糖及血脂代谢异常的糖尿病患者的牙周炎症加重,二者之间相互影响。 Objective To study the correlation between blood lipids and inflammatory factors of type 2 diabetes mellitus (T2DM)with moderate to severe periodontitis. Methods A total of 53 patients with T2DM treated in Department of Endocrinology of the Affiliated Hospital of Southwest Medical University from May 2013 to May 2014 were divide into T2DM with moderate to severe periodontitis group (group A, n=26), simple T2DM group (group B, n=27) ; in the same period, the patients accepted the Affiliated Stomatological Hospital of Southwest Medical University were divide into periodontitis group (group C, n=25 ), healthy control group (group D, n =25 ).The state of periodontal [bleeding on probing (BOP), probing depth (PD), attachment loss (AL) ] were recorded. The fasting blood glucose (FPG) and blood lipids[total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C)]and inflammation factors[ high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor α (TNF-α)]were detected. Results The differences of PD and AL among in four groups were significant (P〈0.05). The differences of FPG, TC, TG, LDL-C, HDL-C, hs-CRP,TNF-α among in four groups were significant (P〈0.05). There was a positive correlation between FPG, TG, TC, LDL-C, hs-CRP, TNF-α and PD, AL, respectively (P〈0.05), but there was a negative correlation between HDL-C and PD, AL, respectively (P〈0.05). Conclusion Moderate to severe periodontitis aggravate the happen of T2DM, glucose and lipid metabolism abnormality of T2DM aggravate periondontal inflammation, them between the interaction.
