梅毒性葡萄膜炎1例

A Case of Syphilitic Uveitis

患者女,59岁,因双眼视力下降于眼科就诊,眼底荧光造影提示双眼葡萄膜炎,行甲苯胺红不加热血清试验(TRUST)为1∶32(+),梅毒螺旋体颗粒凝集试验(TPPA)(+)。诊断:梅毒性葡萄膜炎。患者拒行脑脊液检查,予双侧球旁甲强龙20mg注射及苄星青霉素240万U两臀肌注,1次/周,共4次治疗。治疗结束后患者双眼视力明显好转,3个月后复查血TRUST 1∶16,半年后为1∶8,1年后TRUST转阴。

A 59-year-old female patient,who had binocular vision impairment due to ophthalmologic treatment,fundus fluorescein angiography prompted binocular uveitis,blood detection revealed TRUST 1∶ 32( +),TPPA( +).Diagnosis: syphilitic uveitis. The patient refused to take cerebrospinal fluid examination. Given the methylprednisolone 20 mg injected next to the bilateral ball and benzathine penicillin 240 million U two intramuscular injection,once a week for four weeks. After treatment,binocular vision improved significantly,after 3 months the TRUST was 1∶ 16,after 6 months TRUST was 1∶ 8,after 1 year the TRUST was negative.
