丘疹坏死性结核疹1例

A Case of Papulonecrotic Tuberculid

患者女,65岁。全身皮疹伴发热20余天。皮损组织病理示:真皮全层至皮下脂肪大量嗜中性粒细胞、淋巴细胞、组织细胞浸润,散在多核巨细胞、嗜酸性粒细胞及核尘,纤维素样坏死可见,部分血管内皮细胞肿胀,管壁增厚,伴红细胞外溢。PPD试验强阳性。诊断:丘疹坏死性结核疹。

A 65-year-old female came to our hospital for the lesions on the whole body with fever for 20 days. The histo- pathology of the skin lesions revealed that lots of inflammatory ceils infiltrated in the dermis and subcutaneous fat tissue, including neutrophils, lymphocytes and histiocytes, scattered multinucleated giant cells, eosino- phils, as well as nuclear dusts, fibrinoid necrosis could be seen. Parts of the vascular endothelial cells swelled and vascular walls were thickened, with extravasation of erythrocytes. PPD test was strongly posi- tive. Diagnosis of papulonecrotic tuberculid was made.
