界限类偏瘤型麻风1例

A Case of Borderline Lepromatous Leprosy

患者女,59岁。全身红斑、四肢麻木半年,临床表现为躯干、四肢见大小不等,椭圆形、梭形及不规则暗红色环状红斑。皮损病理示:真皮全层血管、附属器和神经周围大量淋巴细胞、组织细胞浸润和泡沫细胞团块状浸润。抗酸染色(5+)。皮肤切刮查菌:大腿(2+),腹部(+),背部(+)。诊断为界限类偏瘤型麻风。

A 59-year-old woman presented with widespread erythema and limb numbness for six months, the clinical mani- festations showd different size, oval, fusiform and irregular dark red annular erythema around trunk and limbs. Histopathology showed many foamy histiocytes and lymphocytes surrounding vessels, nerves and appendages in whole dermis. Acid-fast staining showed （5 ＋ ）. Skin bacteria search： thigh （2 ＋ ）, abdomen （ ＋ ）, back （ ＋ ）. The diagnosis was borderline lepromatous leprosy.
