丹参多酚酸盐、压力袜联合低分子肝素钙预防妇科术后下肢深静脉血栓的临床研究

Clinical research on salvianolate,compression stockings combined with low molecular weight heparin calcium in the prevention of deep venous thrombosis after gynecologic operation

目的探讨采用丹参多酚酸盐、压力袜联合低分子肝素钙预防妇科术后下肢深静脉血栓的临床疗效。方法选择2015年12月~2016年12月在我院行妇科手术患者84例,按照随机原则分为两组,其中对照组采用丹参多酚酸盐静滴、压力袜方案预防治疗,观察组则是采用丹参多酚酸盐、压力袜联合低分子肝素钙预防措施,两组患者凝血相关指标、血小板术前术后的变化情况比较,同时行下肢静脉超声检查统计下肢深静脉血栓的发生率。结果两组患者术前、术后的凝血功能相关指标及血小板指标的水平情况,比较在术前两组患者的各项指标并无统计学意义(P>0.05),而在术后,观察到两组患者的各项指标相比术前有了较为明显的改善,差异有统计学意义(P<0.05),同时观察组患者的术后各项指标相比对照组而言,其D-二聚体指标水平要明显低于对照组,差异有统计学意义(P<0.05),其余指标无统计学意义(P>0.05)。在下肢深静脉血栓发生率对比中,观察组发生率为2.38%(1/42),而对照组发生率则高达23.81%(10/42),差异有统计学意义(P<0.05)。结论对于需要行妇科手术的患者,可以应用丹参多酚酸盐、压力袜联合低分子肝素钙进行预防治疗,不仅能够有效改善凝血状态,同时还可以显著降低下肢深静脉血栓的发生率,安全性更高,值得临床推广。

Objective To explore the clinical effect of salvianolate,compression stockings combined with low molecular weight heparin calcium in the prevention of deep venous thrombosis after gynecologic operation. Methods 84 patients who received gynecologic operations in our hospital from December 2015 to December 2016 were selected and they were randomly divided into two groups.The control group was used intravenous drip of salvianolate combined with compression stocking while the observation group was used salvianolate,compression stockings combined with low molecular weight heParin calcium. Results Coagulation-related indexes and platelet index before and after operation of patients in two groups were compared.Before operation,every index of patients in two groups had no statistical difference （P 〉 0.05）.After operation,every index of patients in two group was greatly improved compared with that before operation （P 〈 0.05）.In addition,plasma D-dimer of the observation group was significantly lower than that of the control group by comparing every index after operation of two groups.The difference was statistical significant （P 〈 0.05）.Other indexes had no significant statistical difference （P 〉0.05）.In comparison of incidences of deep venous thrombosis,the incidence of the observation group was 2.38% （1/42） and that of the control group was as high as 23.81% （10/42）.The difference was statistical significant （P 〈 0.05）. Conclusion For patients undergoing gynecologic operation,salvianolate,compression stockings combined with low molecular weight heparin calcium to give preventive treatment can not only effectively improve coagulation state,but also significantly decrease the incidence of deep venous thrombosis.h is more safe and worthy of clinical promotion.
