早产儿血管瘤治疗的临床分析

Clinical analysis of treatment of premature infants with hemangioma

目的探讨分析并总结早产儿血管瘤的治疗方法。方法回顾分析2014年5月~2015年5月广东省妇幼保健院医学美容科82例早产儿血管瘤的临床资料,根据血管瘤的类型和分期进行治疗。10例外用噻吗洛尔滴眼液,18例行双波长激光脉冲染料激光治疗+外用噻吗洛尔滴眼液,20例行口服普萘洛尔片+外用噻吗洛尔滴眼液,6例行平阳霉素药物注射,28例行口服普萘洛尔片+平阳霉素药物注射+外用噻吗洛尔滴眼液。结果所有病例经过6个月~2年随访,总有效率85.36%,不良反应轻微,主要包括皮肤质地改变、睡眠障碍、胃肠道反应、及术后水疱、萎缩性瘢痕等。结论早产儿血管瘤,需尽早治疗,对于不同类型、不同时期的早产儿血管瘤,采用合适的联合治疗方法,及时调整治疗方案,是提高治愈率及减少并发症的关键。

Objective To analyze and summarize the treatment of hemangioma in preterm infants. Metnods Clinical data of 82 cases of premature infants with hemangioma in Department of Medical Cosmetology of Guangdong Women and Children Hospital from May 2014 to May 2015 were retrospectively analyzed.According to the type,they were given stage of hemangioma treatment. 10 cases were givon timolol eye drops for external use,18 cases were treated with dual wavelength laser pulse dye laser therapy＋timolol eye drops for external use,20 cases were treated with oral propranolol tablets＋timolol eye drops for external use,6 cases were treated with Pingyang drug injection,and 28 cases were treated with oral propranolol tablets＋Pingyang drug injection＋ timolol eye drops for external use. Results All cases were followed up for six months to two years,the total effective rate was 85.36%,and the adverse reactions were mild,including skin texture changes,sleep disorders,gastrointestinal reactions,and postoperative blisters,atrophic scars,etc.. Conclusion Premature infant hemangioma needs to be treated as soon as possible.For different types of different stages of early childhood hemangioma,the use of appropriate combination therapy and adjust the treatment plan in time are the key to improve the cure rate and reduce complications.
