富含半胱氨酸的酸性分泌蛋白、基质金属蛋白酶-2在结直肠癌和结直肠腺瘤中的表达及其临床意义

Expression and clinical significance of secreted protein acidic and rich in cysteine and matrix metalloproteinase-2 in human colorectal carcinoma and colorectal adenoma

目的研究富含半胱氨酸的酸性分泌蛋白（SPARC）和基质金属蛋白酶（MMP）-2在结直肠癌和结直肠腺瘤中的表达、临床意义及两者之间的关系。方法采用免疫组化SABC法检测65例结直肠癌、32例结直肠腺瘤（腺瘤直径均≥1Cnl）及29例正常结直肠组织中SPARC及MMP-2的表达，并分析其相关性。结果免疫组化结果显示SPARC和MMP-2在结直肠腺管细胞及间质细胞中均有不同程度表达。结直肠癌、结直肠腺瘤及正常组织三者间比较，SPARC在腺管细胞中的阳性表达率无明显差异（P〉0．05），而在间质细胞中阳性表达率的差异有统计学意义（P〈0．05）；MMP-2在腺管细胞中阳性表达率的差异有统计学意义（P〈0．05），而间质细胞中MMP．2阳性表达率差异无统计学意义（P〉0．05）。在结直肠腺癌组织中，腺管细胞中MMP-2的表达与间质细胞中SPARC的表达呈明显负相关（r=-0．465，P〈0．001），而与腺管细胞中SPARC的表达无明显相关（r=0．156，P=0．214）。结论SPARC及MMP-2在结直肠腺癌和结直肠腺瘤中的表达存在差异，提示两者可能在大肠肿瘤的发生、发展、浸润、转移过程中发挥重要作用。

Objective To investigate the expression and clinical significance of secreted protein acidic and rich in cysteine （SPARC）and matrix metalloproteinase-2 （MMP-2）in human colorectal carcinoma and colorectal adenoma and their relationship. Methods The expression of SPARC and MMP-2 in 65 cases colorectal carcinoma, 32 cases colorectal adenoma and 29 cases normal colorectal tissues were detected by the immunohistochemical SABC technique, then to compare the results and analyze their correlation. Results Immunohistochemical results showed that SPARC and MMP-2 expressed in different degree in gland cells and stromal cells of colorectum. Compared with colorectal cancer, cdorectal adenoma and normal tissue, the positive expression rates of SPARC in the gland cells had no significant difference（ P 〉 0.05 ）, while the difference was clear in stromal ceils （ P 〈 0.05 ）, but the expression rates of MMP-2 in the gland cells and stromal cells were opposited. In tissues of colorectal cancer,the expression of MMP-2 in gland cells was negatively correlated with the expression of SPARC in stromal cells（ r = - 0. 465 ,P 〈0. 001 ） ,but were not correlated with the expression of SPARC in gland cells（r =0. 156,P =0.214）. Conclusion The expression of SPARC and MMP-2 in colorectal carcinoma and colorectal adenoma are different which indicates they play an important role in pathogenesis, differentiation,invasion and metastasis of colorectal tumour.
