超声造影和宝石能谱CT对胃肠道肿瘤诊断临床价值分析

Comparison of contrast-enhanced ultrasonography and gemstone spectroscopy CT on the diagnosis of gastrointestinal tumors

目的探讨超声造影和宝石能谱CT对胃肠道肿瘤疾病的诊断价值。方法对我院300例胃肠道肿瘤患者采用胃肠超声造影检查,然后再行宝石能谱CT检查,统计对比两种诊断方式不同胃肠道肿瘤疾病类型诊断敏感度及特异度、胃肠道恶性肿瘤TNM分期诊断准确率。结果宝石能谱CT胃间质瘤、胃淋巴瘤、大肠淋巴瘤、胃体部胃黏膜巨大肥厚症、幽门肥厚症诊断准确度与特异度(93.62%、94.86%)、(84.21%、95.37%)、(87.50%、94.72%)、(93.10%、95.57%)、(89.29%、97.06%)与超声造影检查[(95.74%、94.07%)、(73.68%、94.31%)、(81.25%、95.07%)、(86.21%、96.68%)、(92.86%、97.79%)]比较,差异无统计学意义(P>0.05),宝石能谱CT胃癌及大肠癌诊断准确度与特异度(90.15%、94.64%)、(89.66%、93.36%)高于超声造影[(76.52%、79.17%)、(62.07%、87.08%)],差异有统计学意义(P<0.05);宝石能谱CT检查胃肠道恶性肿瘤TNM分期诊断准确率86.34%高于超声造影77.02%,差异有统计学意义(P<0.05)。结论宝石能谱CT及超声造影在胃肠道肿瘤疾病诊断中具有较高价值,且宝石能谱CT对恶性肿瘤及其TNM分期诊断准确度更高,联合两种检查方式实施诊断,可提高胃肠道肿瘤疾病诊断准确度。

Objective To evaluate the diagnostic value of contrast-enhanced ultrasonography and gemstone spectroscopy CT in gastrointestinal neoplasms. Methods From January 2014 to December 2015,300 patients with gastrointestinal cancer in our hospital were analyzed, and all of them were received by contrast-enhanced ultrasonography. We then checked the gemstone spectrum CT, the different gastrointestinal cancer disease type diagnostic sensitivity and specificity. Gastrointestinal malignancy TNM staging diagnostic accuracy of two diagnostic methods were statistically compared. Results In compar/sion to the diagnostic accuracy and specificity of gemstone spectrum CT in gastric stromal tumor, gastric lymphoma, colorectal lymphoma, gastric body hypertrophy of gastric mucosa, pyloric hypertrophy （93.62%, 94.86% ）, （84.21%, 95.37% ）, （87, 50% ,94.72% ）, （93.10%, 95.57% ）, （89. 29%, 97.06% ）, and contrast-enhanced ultrasonography （95.74%, 94. 07% ）, （73.68%, 94.31% ）, （81.25%, 95.07% ）, （86.21%, 96.68% ）, （92.86%, 97.79% ）, the difference was not statistically significant （ P 〉 0. 05）. The diagnostic accuracy and specificity of gemstone spectroscopy CT in gastric and colorectal cancer （89. 62%, 94. 64% ）, （ 89. 66%, 93.36% ） were significantly higher than those of contrast-enhanced ultrasonography （ 76. 52%, 79. 17% ）, （62.07%, 87.08% ） （ P 〈 0.05 ） ; The diagnostic accuracy of gemstone spectroscopy CT gastrointestinal malignancy TNM staging 86.34% （ 139/161 ） was higher than that of contrast-enhanced uhrasonography 77.02% （ 124/161 ）, the difference was statistically significant （ P 〈 0.05）. Conclusion Gemstone spectroscopy CT and contrast-enhanced uhrasonography have a high value in the diagnosis of gastrointestinal neoplasms, and the diagnostic accuracy of gemstone spectrum CT in the malignant tumors and TNM staging is higher, the diagnostic accuracy of gastrointestinal cancer disease can be further improved by combined with two kinds of inspection methods.
