医护一体化品管圈护理在胸外科管道护理中的临床效果探析

The effect of medical care integrated quality control circle nursing in the tube nursing of thoracic surgery

目的探讨医护一体化品管圈护理运用于胸外科管道护理中对降低非计划性拔管、护理工作满意度评分和不良事件的影响。方法选择2014年10月至2015年10月收治的胸外科住院患者72例作为常规组，给予一般管道护理；再选择2015年11月至2016年11月收治的胸外科住院患者72例作为干预组，给予医护一体化品管圈活动的管道护理。观察并记录两组非计划性拔管发生率、护理工作满意度评分和管道不良事件情况，展开比较分析。结果干预组非计划性拔管发生率为1．39％，低于常规组的15．28％，差异有统计学意义（P〈0．05〉干预组护理人员工作满意度评分为（3．98±0．61），医生工作满意度评分为（4．57±0．56），优于常规组的（2．97±O．46）和（2．72±0．41），差异有统计学意义（P〈0．05）；干预组管道不良事件发生率为1．79％，低于常规组的18．06％，差异有统计学意义（P〈0．05）。结论医护一体化品管圈护理运用于胸外科管道护理中可明显降低非计划性拔管发生率，减少不良事件发生，改善护理工作质量，值得临床推广。

Objective To explore the effect of medical care integrated quality control circle nursing in reducing unplanned extubation, nursing work satisfaction score, and the incidence of adverse events in the tube nursing of thoracic surgery. Methods Seventy-two inpatients in thoracic surgery admitted from October 2014 to October 2015 were selected as routine group, who received general tube nursing. Seventy-two inpatients in thoracic surgery admitted from November 2015 to November 2016 were selected as intervention group, who received medical care integrated quality control circle nursing. Observed and recorded the incidence of unplanned extubation, nursing work satisfaction score, and the incidence of adverse events in the two groups. Results The incidence of unplanned extubation in the intervention group was 1.39%, significantly lower than 15.28% in the routine group （P〈0.05）. The nursing work satisfaction score and the doctor＇s work satisfaction score of the intervention group were （3.98~0.61） and （4.57~0.56）, better than （2.97~0.46） and （2.72~0.41） of the routine group, with statistically significant differences （P〈0.05）. The incidence of adverse events in the intervention group was 1.79%, significantly lower than 18.06% in the routine group （P〈0.05）. Conclusion Medical care integrated quality control circle nursing applied in the tube nursing of thoracic surgery, can reduce the incidence of unplanned extubation and the occurrence of adverse events, improve the quality of nursing care, worthy of clinical popularization.
