摘要
目的检验环状软骨超声解剖直径能否作为临床气管插管术气管导管型号选择的依据。方法 25例需要在气管插管全身麻醉下完成手术患者,ASAⅠ~Ⅱ级,采用超声技术测量其环状软骨横截面空气的左右直径。采用最小二乘法对环状软骨超声解剖直径与临床麻醉所需的最佳气管导管直径进行回归分析;采用Bland-Altman法分析环状软骨超声解剖直径与临床麻醉最佳气管导管直径一致性。结果 (1)环状软骨超声下解剖直径与最佳气管导管直径之间存在线性关系,且具有良好的相关性,回归方程为Y=2.57+0.59X,R^2=0.97。(2)环状软骨超声下解剖直径与最佳气管导管直径配对数据差值的均数d=2.59 mm,差值的标准差Sd=0.63 mm,95%一致性界限为(1.35 mm,3.83 mm)。结论环状软骨超声解剖直径可以作为气管插管术的气管导管型号选择依据。
Objective To test whether the cricoid cartilage diameter by Ultrasound anatomy can be used as the basis for the selection of endotracheal tube type in clinical tracheal intubation. Methods 25 patients (ASA Ⅰ -Ⅱ ) with tracheal intubation under general anesthesia were enrolled. The left and right diameters of the cricoid cartilage transverse section were measured by ultrasonography. The regression? analysis? for the diameter of cricoid cartilage by Ultrasound anatomy and optimum endotracheal tubes for clinic anesthesia was carried out by the least square method. The consistency of the diameter of cricoid cartilage by Ultrasound anatomy and optimum endotracheal tubes for clinic anesthesia were analyzed by the Bland -Altman method. Results There was a linear relationship and good correlation between the diameter of cricoid cartilage by Ultrasound anatomy and optimum endotracheal tubes. The regression equation was Y =2.57 +0.59X,R2 =0.97. The D -value average of cricoid cartilage diameter by Ultrasound anatomy and optimum endotracheal tubes diameter was J =2.59mm and D-value standard deviation was Sd =0.63mm (95% consistency limits = 1.35mm, 3.83mm). Conclusion The cricold cartilage diameter by Ultrasound anatomy could be used as the basis for the selection of endotracheal tube type in clinical tracheal intubation.
