患者-家庭-医院一体化干预治疗对COPD患者生活质量及肺功能的影响

EFFECT OF PATIENT-FAMILY-HOSPITAL INTEGRATED INTERVENTION ON THE QUALITY OF LIFE AND LUNG FUNCTION IN PATIENTS WITH COPD

目的探讨患者-家庭-医院一体化干预治疗对COPD患者生活质量及肺功能的影响。方法将2013年8月—2016年8月在华北理工大学附属医院接受住院治疗的COPD患者154例作为研究对象。将以上研究对象按随机数字表法随机分成对照组和观察组,其中对照组76例,观察组78例。给予对照组患者COPD规范化治疗,而观察组除给予规范化药物治疗外,采用患者-家庭-医院一体化干预治疗模式,应用CAT量表及肺功能检查对二组患者生活质量及肺功能情况进行对比评价。结果观察组治疗后CAT评分明显较治疗前下降(P<0.05),具有统计学意义;对照组治疗后CAT评分较治疗前虽有所下降,但不明显,差异无统计学意义(P>0.05);二组肺功能检查1s用力呼气容积(FEV1)治疗前后无明显改善,差异无统计学意义(P>0.05),但其占预计值的百分比(FEV1%pre)观察组较对照组提高,具有统计学意义(P<0.05)。结论患者-家庭-医院一体化干预治疗可提高COPD患者生活质量,在一定程度上改善肺功能,提高患者对治疗的信心。

Objective To investigate the effect of Patient- Family- Hospital integrated intervention on the quality of life and lung function in patients with COPD. Methods A total of 154 patients with COPD admitted to author＇s hospital from August 2013 to August 2016 were enrolled in the study. The above research subjects were randomly divided into control group and observation group, 76 cases in the control group, the observation group of 78 cases. The control group received COPD standardized treatment, while the observation group received standardized drug treatment, patients with family - hospital integrated intervention model, using the CAT scale and pulmonary function examination were performed to evaluate the two groups of patients quality of life and lung function. Results The observation group after treatment CAT score was significantly lower than that before treatment （P〈0.05）, with statistical significance; the eontrol group after treatment than before treatment CAT score has decreased, but not significantly （P〉 0.05）, the two groups had no statistical significance. Lung function forced expiratory volume （FEV1） before and after treatment had no obvious improvement （P〉0. 05）, but the percentage of predicted value （FEV1% pre） was increased in the observation group than the control group （P〈0.05）, with statistical significance. Conclusion Patient family hospital integrated intervention can improve the quality of life of patients with COPD, to a certain extent, improve lung function, improve patient confidence in treatment.
