系统性股动脉穿刺教学法在心血管内科的应用

APPLICATION OF SYSTEMATIC METHOD OF FEMORAL ARTERY PUNCTURE TRAINING IN DEPARTMENT OF CARDIOLOGY

摘要 股动脉穿刺技术是介入心脏病学的基础操作技能,是心血管内科医生学习介入诊治技术的基本功,即使在经桡动脉穿刺冠心病介入诊治方式大行其道的背景下,这项古老的技术依然具有它不可替代的地位和作用。同时也是外周血管穿刺技术中最容易出现并发症的操作,是学习和实践介入心脏病学的第一道门槛。学习好、使用好这项技术,能够为介入操作奠定良好的有效性和安全性基础;相反,如果穿刺没做好,即使心脏操作做好了,也可能给患者带来病痛甚至严重的健康或生命危害。本文从实践出发,总结出一套系统性股动脉穿刺教学法,并应用到临床带教中去,从实际带教效果中可以看出,本方法能够使学员逐步稳定地掌握穿刺技能,在带教老师的引导下,安全地实施操作并有序获得独自操作的能力。在其后的临床实践中,还能根据本方法进行技术水平自我评估,逐步改善提高穿刺的准确度和把握度。 Even though that transradial artery PCI have been become routine choice in catheterlab nowdays, the old technique of femoral artery puncture maintains its unique status and usefulness by way of being basic procedure and basic training for interventional cardiology. The procedure of femoral artery puncture, most often being considered as the first doorsill for learning and practice interventional cardiology, may most likely induce complications among peripheral vascular punctures. Well performance of the femoral artery puncture constitutes an effective and safe base for the whole procedure. Otherwise, a failure puncture may hazard the health or even life of the patient in spite of the success of heart procedure. According to the practice in our center, a systematic teaching method of femoral artery puncture was established and applied to practice. From the initial results of training, the method can make trainees master the puncture skill gradually and do the job safely by the guidance of trainer. And from then on, trainees can evaluate themselves and acquire constant increase in their performance skills by support of the method.
