实时荧光定量PCR检测儿童人巨细胞病毒感染的临床意义 被引量:9

Clinical significance of qRT-PCR in the detection of children with human cytomegalic inclusion disease virus infection

摘要 目的应用实时荧光定量PCR(qRT-PCR)技术检测人巨细胞病毒DNA(HCMV DNA),探讨对儿童HCMV感染的早期临床诊断的意义。方法选择2012年1月-2015年12月来本院就诊的疑似HCMV感染的1 301例门诊和住院患儿,采用qRT-PCR技术对血清标本进行HCMV DNA的定量检测。结果 HCMV DNA检测总阳性率为8.84%;男性和女性阳性率分别为8.46%和9.46%,差异无统计学意义(χ~2=0.38,P>0.05)。<1岁组的阳性率最高,为10.88%;其次是1岁~组(1.75%),3岁~14岁组最低,为1.2%,各年龄组之间差异有统计学意义(χ~2=24.487,P<0.05)。临床诊断主要以黄疸、巨细胞病毒感染、肝损害、肺炎为主。结论≤1岁组的儿童易感染人巨细胞病毒,加强对HCMV DNA的检测,有助于临床早期诊断和治疗,对预防肝、肺等器官并发症有重要的临床意义。 Objective To explore the early clinical diagnostic value of human cytomegalovirus (HCMV) infection in children by fluorescent quantitative real time -polymerase chain reaction( qRT -PCR) to detect HCMV DNA. Methods Serum specimens were collected form 1 301 outpatient and inpatient children with HCMV infection from January 2012 to December 2015, using qRT - PCR for the quantitative detection of HCMV DNA. Results Total positive rate of HCMV DNA was 8.84%. The positive rates of male and female samples were 8.46% and 9.46%, respectively, and the differences were not statistically significant (X2 = 0.38, P 〉 0.05 ). The positive rate in children aged less than 1 year accounted for 10.88%, followed by the positive rate of older than 1 year group( 1.75% ). The lowest positive rate was 1.2% in patients aged 3 - 14, and the differences in all age groups were statistically significant (X2 = 24. 487, P 〈 0.05). Conclusion Children aged less than 1 year were more suscepti- ble to HCMV. We should strengthen the detection of HCMV DNA to help clinical early diagnosis and treatment, and it has im- portant clinical significance to prevent complications in liver, lung and other organs.
