石家庄市鹿泉区肺结核病人就诊延迟情况及相关因素分析

Analysis of delay and related factors in patients with pulmonary tuberculosis in Luquan District of Shijiazhuang

目的了解石家庄市鹿泉区肺结核病人就诊延迟情况,探索与就诊延迟相关的主要因素。方法收集石家庄市鹿泉区2013-2015年登记的结核病病人的个案资料,用描述性方法分析就诊延迟率,并对就诊延迟情况及相关因素进行统计学分析。结果石家庄市鹿泉区肺结核病人就诊延迟率为32.86%,就诊延迟中位数为31 d,20.63%就诊延迟在1个月以上。性别、年龄和病人来源与就诊延迟相关,女性就诊延迟高于男性(OR=1.504,95%CI=1.018~2.223);15~29岁组低于其他年龄组(OR=1.940,95%CI=1.215~3.097);因症就诊病人就诊延迟高于转诊、追踪(OR=0.192,95%CI=0.072~0.512);因症推荐病人就诊延迟高于转诊、追踪病人(OR=0.147,95%CI=0.029~0.740)。结论石家庄市鹿泉区肺结核病人就诊延迟率较高,就诊延迟与性别、年龄和来源相关。建议采取综合有效措施,减少肺结核病人就诊延迟现象。

Objective To understand the delay of treatment in patients with pulmonary tuberculosis in Luquan District of Shiji- azhuang City, and to explore the main factors related to the delay of treatment. Methods The case data of tuberculosis patients registered in Luquan District from 2013 to 2015 were collected, and the delay rate of treatment was analyzed by using descriptive analysis. The delay and related factors of patients with tuberculosis were statistically analyzed. Results The delay rate of pulmo- nary tuberculosis patients in Luquan District was 32. 86%, the median treatment delay was 31 d, and 20.63% of patients with pulmonary tuberculosis were delayed in 1 months or more. Gender, age, and derived from the patient and treatment delay, fe- male doctor delay than men （ OR = 1. 504, 95% CI = 1. 018 - 2. 223 ） ; 15 N 29 year old age group is lower than other age groups （ OR = 1. 940, 95 % CI = 1. 215 - 3.097 ） ; for patients with patient delay is higher than that of referral and tracing patients （ OR = 0. 192, 95 % CI = 0. 072 - 0. 512） ; disease as a resuh of recommended patient delay is higher than that of referral and tracing patients （ OR = 0. 147, 95 % CI = 0. 029 N 0. 740 ）. Conclusion Patients with tuberculosis in Luquan District were associated with a higher rate of treatment delay, delay in treatment and gender, age and source. It is recommended to take comprehensive effective measures to reduce the delay in treatment of patients with tuberculosis in the area.
