无创通气治疗对慢阻肺合并Ⅱ型呼吸衰竭患者的临床疗效评价

Clinical evaluation of noninvasive ventilation in treatment of COPD patients complicated with respiratory failure

目的研究评价无创通气治疗对慢阻肺合并Ⅱ型呼吸衰竭患者的临床疗效。方法试验中选取我院2014年7月至2016年11月诊治的76例慢阻肺合并Ⅱ型呼吸衰竭患者,将全部患者随机分成研究组与对照组,各38例;其中对照组在治疗中实施常规、对症等治疗,研究组在对照组的治疗基础上给予患者无创通气治疗,观察分析组间的治疗效果及血气指标变化情况等。结果全部入选患者通过治疗后,对患者的生命体征及血气指标变化情况进行对比分析表明,研究组HR、呼吸频率、PaO_2、PaCO_2等指标均明显优于对照组(P<0.05),且研究组的插管率、住院时间及病死率等预后指标较对照组均明显减少(P<0.05)。结论慢阻肺合并Ⅱ型呼吸衰竭患者在治疗中应用无创通气治疗,临床疗效显著,且安全可靠,具有较高的临床应用价值。

Objective To study the evaluation of noninvasive ventilation therapy for copd combined type Ⅱ the clinical curative effect of patients with respiratory failure. Methods 76 cases of COPD patients with type Ⅱ respiratory failure treated in our hospital from July 2014 to November 2016 were selected, all patients were randomly divided into research group and the control group, each group with 38 cases; The control group was treated with routine treatment and symptomatic treatment, while the control group was treated with noninvasive ventilation, and observed the treatment effect and the change of blood gas index among the group. Results The HR, respiratory rate, PaO2, PaCO2 and other indicators in the study group were significantly better than those in the control group （P〈0.05）, and the intubation rate, hospitalization time and mortality rate of the study group were significantly lower than those of the control group （P 〈 0. 05）. Conclusions Noninvasive ventilation in treatment of COPD patients complicated with respiratory failure, which clinical efficacy is significant, safe and reliable, and has high clinical application value.
