自我管理小组在社区高血压病患者血压管理中的应用

Application of self-management team in blood pressure management of community hypertensive patients

目的:探讨自我管理小组在社区高血压病患者血压管理中的应用效果。方法:选取2015年6~12月社区健康服务中心确诊为高血压病患者92例为对照组,接受常规护理管理;另选同期92例确诊为高血压病患者为观察组,采用自我管理小组方法进行护理管理。对两组患者自我管理能力、服药依从性、管理满意度等进行观察。结果:观察组患者自我管理能力各维度评分均较对照组高,两组差异有统计学意义(P<0.05);观察组1年后的服药依从率为92.39%,管理满意率为95.65%,对照组分别为55.43%、68.48%,两组比较差异有统计学意义(P<0.05)。结论:自我管理小组提高社区高血压病患者自我管理能力,也能提高患者服药依从性与管理满意度,对改善社区高血压病患者生存质量,降低并发症发生风险具有重要价值,值得进一步推广。

Objective：To explore the application effect of self - management team in blood pressure management of community hypertensive patients. Meth-ods ：A total of 92 patients with hypertension who were confirmed with hypertension in the community health service center from June 2015 to December 2015 was selected as the control group. The patients were treated with conventional nursing management. A total of 92 patients with hypertension were selected as observation group in the same period and they were treated with self - management team care management. Self - management ability, medication compli-ance and management satisfaction of the the two groups were observed. Results ： The scores of self - management ability of the observation group were higher than those of the control group. The difference between the two groups was statistically significant（ P 〈0. 05）. Medication compliance rate of the observation group after 1 year was 92. 39% and the management satisfaction rate was 95. 65% . The data of the control group were 55.43% and 68.48% respectively. The difference between the two groups was statistically significant（ P 〈 0.05 ）. Conclusion ： Self - management team can not only improve the self - manage-ment ability of patients with hypertension in the community, but also improve the patient’s compliance and management satisfaction. It is of great value to improve the quality of life and reduce the risk of complications in patients with hypertensive. Therefore, it is worthy of further promotion.
