妊娠期糖尿病患者血糖水平对妊娠结局的影响

Effect of Blood Glucose Level on Pregnancy Outcome in Patients with Gestational Diabetes Mellitus

目的探讨妊娠期糖尿病患者血糖水平对妊娠结局的影响。方法选取2015年3月至2016年11月于我院住院分娩的妊娠期糖尿病患者79例为研究对象,根据患者血糖控制结果分为研究组(44例,控制有效)和对照组(35例,控制无效),比较两组患者的妊娠不良结局发生率。结果研究组产妇的剖宫产、羊水异常、胎膜早破、产后出血、妊娠期高血压及产褥感染发生率均显著低于对照组,差异有统计学意义(P<0.05)。研究组的新生儿巨大儿、胎儿窒息、胎儿窘迫、低血糖及高胆红素血症发生率均显著低于对照组,差异有统计学意义(P<0.05)。结论妊娠期糖尿病患者的血糖水平对产妇及新生儿的相关并发症均有显著影响,临床中应严格控制妊娠期糖尿病患者的血糖水平,以降低妊娠不良结局的发生率,保障母婴健康。

Objective To investigate the effect of blood glucose level on gestational outcome in patients with gestational diabetes mellitus. Methods 79 cases of patients with gestational diabetes mellitus in our hospital form March 2015 to November 2016 were divided into study group （n = 44, effective control） and control group （n = 35, invalid control） according to the results of blood glucose control. The incidences of adverse pregnancy outcomes of two groups were compared. Results The incidences of cesarean section, amniotic fluid abnormalities, premature rupture of membranes, postpartum hemorrhage, pregnancy induced hypertension and puerperal infection of study group were significantly lower than those of control group （P 〈0.05）. The incidences of neonatal macrosomia, fetal asphyxia, fetal distress, hypoglycemia and hyperbilirubinemia of study group were significantly lower than those of control group （P〈0.05）. Conclusions The level of blood glucose of patient with gestational diabetes mellitus has significant influence on the maternal and neonatal complications, which suggests that the blood sugar levels of patients with gestational diabetes mellitus should be strictly controlled, so as to reduce the incidences of adverse pregnancy outcomes, and protect the maternal and neonatal health.
