摘要
目的研究儿童重症肺炎患儿凝血指标与降钙素原的关系及意义。方法选取2014年4月至2016年4月我院收治的重症肺炎患儿50例作为实验组,另外选取50例健康儿童作为对照组,检测并比较两组的降钙素原(PCT)、D-二聚体(DD)、血小板计数(PLT)和抗凝血酶Ⅲ(AT-Ⅲ)水平。同时将实验组患儿根据PCT升高的程度分为PCT<2.00 ng/mL组(n=27)和PCT≥2.00 ng/mL组(n=23),比较两组的DD、PLT和AT-Ⅲ水平,以及DIC发生率。结果实验组的PCT、DD、PLT水平均高于对照组,AT-Ⅲ水平显著低于对照组(P<0.05)。PCT<2.00 ng/mL组的PLT、AT-Ⅲ水平均高于PCT≥2.00 ng/mL组,DD水平低于PCT≥2.00 ng/mL组,DIC发生率低于PCT≥2.00 ng/mL组,均P<0.05。结论 PCT水平可以反映重症肺炎患儿的炎性反应发展程度,PCT水平越高,表明病情越严重,凝血功能越差,更易并发DIC。
Objective To study the relationship between coagulation parameters and procalcitonin in children with severe pneumonia and its significance. Methods 50 cases of children with severe pneumonia admitted to our hospital from April 2014 to April 2016 were selected as the experimental group. Another 50 cases of healthy children were selected as the control group. The levels of pocalcitonin (PCT), D-dimer (DD), platelet count (PLT) and antithrombin Ⅲ(AT- Ⅲ) were detected and compared between two groups. According to the level of PCT, children in the experimental group were divided into PCT 〈 2.00 ng/mL group (n = 27) and PCT ≥2.00 ng/mL group (n = 23), and the levels of DD, PLT and AT-Ⅲ, and incidence of DIC were compared between two groups. Results The levels of PCT, DD and PLT of experimental group were significantly higher than those of control group, and the level of AT- Ⅲ was significantly lower than that of control group (P 〈0.05). The levels of PLT and AT-Ⅲ of PCT 〈 2.00 ng/mL group were significantly higher than those of PCT ≥2.00 ng/mL group, the level of DD was significantly lower than that of PCT ≥ 2.00 ng/mL group, and the incidence of DIC was significantly lower than that of PCT ≥ 2.00 ng/mL group (P 〈0.05). Conclusions PCT can reflect the severity of inflammatory response in children with pneumonia. The higher the PCT level, the more serious the illness, the worse the coagulation function, the more likely to complicated with DIC.
