青年冠心病患者血尿酸、脂蛋白磷脂酶A2与动脉僵硬度的相关性分析

Correlation analysis of blood uric acid,lipoprotein associated phospholipase A2 and arterial stiffness in young patients with coronary heart disease

目的 探讨青年冠心病患者血尿酸、脂蛋白磷脂酶A2（Lp-PLA2）与动脉僵硬度的相关性.方法 56例青年冠心病患者作为观察组,50例青年健康查体者作为对照组,测定两组血尿酸、Lp-PLA2.测定观察组患者的动脉僵硬度.分析青年冠心病患者血尿酸、Lp-PLA2与动脉僵硬度的相关性.结果 两组年龄、性别、BMI、收缩压、舒张压、血糖甘油三酯差异无统计学意义（P〉0.05）;观察组总胆固醇、低密度脂蛋白、尿酸、Lp-PLA2均明显高于对照组（t=3.927 4、4.162 4、5.772 6、9.770 1,P〈0.05,均P〈0.01）;观察组高密度脂蛋白明显低于对照组（t=2.604 3,P〈0.05）.Logistic多因素回归分析,结果显示低密度脂蛋白、尿酸、Lp-PLA2为冠心病的独立危险因素（低密度脂蛋白OR=1.053,95%CI 1.031~1.106;尿酸OR=1.082,95%CI 1.043~1.115;Lp-PLA2 OR=1.007,95%CI 1.002~1.011）.观察组动脉僵硬度为（8.86±1.63）v·cm-1·s-1,与血尿酸呈正相关关系（r=0.221,P〈0.05）,与Lp-PLA2呈正相关系（r=0.361,P〈0.05）.结论 血尿酸、Lp-PLA2可以反映早期青年冠心病患者的动脉僵硬度.

Objective To study the correlation of blood uric acid,lipoprotein associated phospholipase A2（Lp-PLA2） and arterial stiffness in young patients with coronary heart disease.Methods 56 young patients with coronary heart disease were selected as the observation group,50 young healthy subjects were selected as control group.The serum uric acid,Lp-PLA2 and arterial stiffness in the two groups were determined.Blood uric acid,Lp-PLA2 and arterial stiffness in patients with coronary heart disease were analyzed.Results There were no significant differences in age,sex,BMI,systolic blood pressure,diastolic blood pressure and triglyceride between the two groups （all P〉0.05）.The total cholesterol,low density lipoprotein,uric acid and Lp-PLA2 in the observation group were significantly higher than those in the control group （t=3.927 4,4.162 4,5.772 6,9.770 1,P〈0.05,P〈0.01）.High density lipoprotein in the observation group was significantly lower than that in the control group （t=2.6043,P〈0.05）.Multivariate Logistic regression analysis showed that low density lipoprotein cholesterol,uric acid,Lp-PLA2 were independent risk factors for coronary heart disease （low density lipoprotein uric acid OR=1.053,95% CI：1.031-1.106;uric acid OR=1.082,95% CI：1.043-1.115;Lp-PLA2 OR=1.007,95% CI：1.002-1.011）.The arterial stiffness of the observation group was （8.86±1.63）v·cm-1·s-1,and had positive correlation with serum uric acid （r=0.221,P〈0.05） and Lp-PLA2 （r=0.361,P〈0.05）.Conclusion Blood uric acid,Lp-PLA2 can reflect the early arterial stiffness in young patients with coronary heart disease.
