摘要
目的研究分析腹腔镜手术麻醉中应用舒芬太尼和芬太尼的麻醉效果。方法方便选择该院2016年2月—2017年2月收治的拟进行腹腔镜手术的患者120例,分为A、B两组,每组60例,给予A组舒芬太尼进行手术麻醉,给予B组芬太尼进行手术麻醉,比较A、B两组患者麻醉效果以及不良反应。结果 A组镇痛效果为Ⅰ级30例(50.0%)、Ⅱ级24例(40.0%)、Ⅲ级6例(10.0%),显著优于B组的Ⅰ级17例(28.3%)、Ⅱ级26例(43.3%)、Ⅲ级17例(28.4%),差异有统计学意义(P<0.05);A组肌肉松弛程度为I级31例(51.7%)、Ⅱ级25例(41.7%)、Ⅲ级4例(6.6%),显著优于B组的I级23例(38.3%)、Ⅱ级20例(33.3%)、Ⅲ级17例(28.4%),差异有统计学意义(P<0.05);A组麻醉后心率、血压均显著低于B组,差异有统计学意义(P<0.05);A组患者治疗效果总有效率为96.0%,与B组的90.0%比较差异无统计学意义(P>0.05);A组不良反应发生几率显著低于B组,差异有统计学意义(P<0.05)。结论综上,舒芬太尼具有镇痛效果显著、麻醉作用良好、不良反应少、预后效果好等特点,安全高效。值得临床大力推广。
Objective To analyze the clinical significance of Sufentanil and Fentanyl in the anesthesia effect during the laparoscopic surgery.Methods 120 patients of adopting the laparoscopic surgery from February 2016 to February 2017 in our hospital were selected.The subjects were randomly divided into two groups,60 cases each group.Group A took Sufentanil;Group B took Fentanyl.The anesthesia effect and adverse event rate for two groups was compared.Result Based on the analgesia effect,Group A had 30 cases with Grade Ⅰ(50.0%),24 cases with Grade Ⅱ(40.0%) and 6 cases with Grade Ⅲ(10.0%);Group B had 17 cases with Grade Ⅰ(28.3%),26 cases with Grade Ⅱ(43.3%) and 17 cases with Grade Ⅲ(28.4%)(P<0.05);based on the muscular flaccidity,Group A had 31 cases with Grade Ⅰ(51.7%),25 cases with Grade Ⅱ(41.7%)and 4 cases with Grade Ⅲ(6.6%);Group B had 23 cases with Grade Ⅰ(38.3%),20 cases with Grade Ⅱ(33.3%) and 17 cases with Grade Ⅲ(28.4%)(P<0.05);after anesthesia,the heart rate and blood pressure for Group A was significantly lower than Group B(P<0.05);the total effective rate for Group A(96.0%) and Group B(90.0%) was not significantly different(P>0.05);the adverse event rate for Group A was significantly lower than Group B(P<0.05).Conclusion Sufentanil has an obvious analgesia and anesthesia effect,reduces the adverse event rate and improves the clinical prognosis and safety.It shall be extensively recommended in clinical practice.
