创面负压治疗对糖尿病足不同缺血创面肉芽组织EDA^+FN的影响 被引量:5

Effect of Negative Pressure Wound Therapy on EDA^+FN Level in Different Ischemia Granulation Tissues of Diabetic Foot Wounds

摘要 目的探讨创面负压治疗(negative pressure wound therapy,NPWT)对糖尿病足不同缺血创面肉芽组织EDA^+FN的影响。方法选取我院2014年1月—2016年1月符合入组标准的糖尿病足30例,依据踝肱比值(ABI),分为0.5≤ABI<0.7组和0.7≤ABI≤0.9组,每组各15例。两组均予NPWT,并于治疗前及治疗第14天取创面肉芽组织,观察EDA^+FN的变化。结果与本组治疗前比较,两组治疗后EDA^+FN面密度值、EDA^+FN相对蛋白表达量及EDA^+FN相对m RNA表达量均显著升高,差异有统计学意义(P<0.01);治疗后0.7≤ABI≤0.9组EDA^+FN面密度值、EDA^+FN相对蛋白表达量及EDA^+FN相对m RNA表达量较0.5≤ABI<0.7组明显升高,差异有统计学意义(P<0.01)。结论 NPWT对糖尿病足不同缺血创面EDA^+FN的影响不同,随着缺血程度的加重,创面肉芽组织EDA^+FN的表达下降。 Objective To investigate effect of negative pressure wound therapy (NPWT) on EDA + FN level in dif- ferent ischemia granulation tissue of diubetic foot wounds. Methods A total of 30 diabetic feet fitting the inclusion criteria during January 2014 and January 2016 were divided into 0.5 ≤ ABI 〈 0.7 group ( n = 15 ) and 0.7≤ ABI ≤0.9 group ( n = 15) according to ABI (ankle brachial ndex) value. All patients received NPWT. Granulation tissues of wounds were collect- ed before treatment and at the 14th d of treatment to observe the changes of EDA- FN. Results Compared With those beforetreatment in same group, values of area density of EDA + FN, relative protein expression of EDA + FN and relative retiNA ex- pression of EDA +FN were significantly increased after treatment in both groups (P 〈 0.01 ). After treatment, values of area density of EDA+ FN, relative protein expression of EDA + FN and relative mRNA expression of EDA + FN in 0.7 ≤ ABI≤0.9 group were significantly higher than those in 0.5 ≤ ABI 〈 0.7 group (P 〈 0. 01 ). Conclusion NPWT has different effects on EDA + FN levels in different ischemia granulation tissue of diabetic foot wounds. EDA+ FN levels in different ischemia granu- lation tissue of diabetic foot wounds may decrease with aggravated isehemia degree.
