摘要 目的:探讨3.0T磁共振(MR)多b值(弥散敏感系数)DWI(弥散加权成像)及动态增强(DCE)在前列腺癌诊断中的应用价值。方法:选择2015年2月至2016年2月经医院穿刺活检疑似为前列腺癌患者84例为研究对象,所有患者均给予MR多b值DWI及DCE-MRI检查,经病理学检查,前列腺癌46例、前列腺增生38例。以术后病理学检查结果为标准,评估MR多b值DWI及DCE-MRI检查前列腺癌的灵敏度、特异度、准确度;比较前列腺癌与前列腺增生患者ADC值与信号曲线类型。结果:DWI及DCE-MRI检查前列腺癌的灵敏度为91.30%、特异度为84.21%、准确度为88.09%:前列腺癌患者ADC值明显低于前列腺增生患者,差异有统计学意义(P<0.05);b值2000s/mm^2时,DWI检测前列腺癌的灵敏度与准确度均明显高于b值1500s/mm^2、1000s/mm^2差异有统计学意义(P<0.05)。结论:3.0T MR多b值DWI及DCE-MRI可显著改善前列腺癌的诊断效果,能够进行有效的鉴别诊断及分期,为临床早期诊治提供合理依据。 Objective: To study the clinical application of DWI (diffusion weighted imaging) and DCE-MRI (dynamic contrast enhanced MRI) in the diagnosis of prostatic cancer. Methods: Through the needle biopsy, 84 suspected patients with prostatic cancer treated from February 2015 to February 2016 in our hospital were selected. All of the subjects received the DWI and DCE-MRI examination. Through the pathological examination, there were 46 cases with prostatic cancer and 38 cases with prostatic hyperplasia. Based on the pathological examination results, the sensitivity, specificity and accuracy based on DWI and DCE-MRI was evaluated. The ADC value and signal curve types for prostatic cancer and prostatic hyperplasia patients were compared. Results: The sensitivity, specificity and accuracy based on DWI and DCE-MRI was 91.30%, 84.21% and 88.09%; the ADC value for prostatic cancer patients was significantly lower than that of prostatic hyperplasia patients (P〈0.05); When the b value was 2000s/mm^2, the sensitivity and accuracy of DWI in detecting prostate cancer were significantly higher than those of b value was 1500s/mm^2 and 1000s/ram^2 (P〈 0.05). Conclusion: The DWI and DCE-MRI examination can improve the clinical diagnosis of prostatic cancer, make an effective differential diagnosis and staging, and provide the reasonable basis in the early treatment.
