摘要 目的探讨MR弥散加权成像(DWI)的量化分析对乳腺癌复发的诊断价值。方法收集临床怀疑乳腺癌根治术后局部复发的病例18例,所有病例均行DWI检查,并分别测量病灶表观弥散系数(ADC)值。结果所有病例均经病理或随访证实,术后局部复发者13例(复发组),无复发者5例(对照组)。复发组感兴趣区ADC值范围(0.373~1.7)×10^(-3) mm^2/s,平均最小ADC值为(0.68±0.24)×10^(-3) mm^2/s。对照组感兴趣区ADC值范围(1.48~2.75)×10^(-3) mm^2/s,平均最小ADC值为(2.07±0.35)×10^(-3) mm^2/s。两组病例的平均ADC值比较,差异有统计学意义(P<0.05)。结论 DWI量化分析对乳腺癌根治术后局部复发有重要意义。 Objective To discuss the value of Quantitative MR diffusion-weighted imaging in the diagnosis of local recurrence after radical mastectomy. Methods To collect the clinical data of 18 patients with local recurrence after radical mastectomy were collected. All cases were examined by DWI and the apparent diffusion coefficient (ADC) values were measured. Results All cases were confirmed by pathology or follow up, 13 cases of local recurrence (recurrence group) were determined, and there was no recurrence in the control group (5 cases). The range of ADC value of the ROI in the recurrent group is (0.373-1.7)×10^-3 mm^2/s, and the average minimum ADC value is (0.68-0.24)×10^-3 mm^2/s. The range of ADC value of the ROI in the control group is (1.48±2.75)×10^-3 mm^2/s, and the average minimum ADC value is (2.07±0.35)× 10^-3 mm^2/s. The difference between ADC value of the two groups of patients was statistically significant (P〈0.05). Conclusion DWI quantitative analysis is of great significance for local recurrence after radical mastectomy.
