赤参壮骨汤治疗骨折延迟愈合及骨折不愈合临床观察

Clinical Observation of Chishen Zhuanggu Decoction was Used to Treat Nonunion and Delayed Union of Fractures

目的:观察中医赤参壮骨汤治疗骨折延迟愈合及骨折不愈合的临床效果。方法:选取2014年1月~2015年12月在我院诊治的72例骨折后延迟愈合或不愈合患者,采用前瞻性研究方法随机分为中医组、常规组各36例,两组均给予骨瓜提取物、高压氧及体外冲击波等基础治疗,中医组同时加用赤参壮骨汤治疗,疗程为4周。结果:治疗前,两组患者的血清ALP、BGP、BMP2、IGF-Ⅰ的水平差异无统计学意义(P>0.05);治疗后,中医组患者的血清ALP、BGP、BMP2、IGF-Ⅰ水平均低于常规组(P<0.05);治疗结束时、治疗结束后1个月、治疗结束后3个月,中医组患者的骨痂形成评分均高于常规组(P<0.05);治疗后6个月,中医组患者的骨折愈合率88.89%高于常规组的66.67%(P<0.05),中医组的平均骨折愈合时间短于常规组(P<0.05)。结论:中医赤参壮骨汤治疗骨折延迟愈合及骨折不愈合能调节骨代谢指标、促进骨痂形成、促进骨折愈合。

Objective： Studying on the effect of using Chishen Zhuanggu Decoction in the treatment of delayed union and nonunion of fracture. Methods ： From January 2014 to December 2015 in our hospital, 72 cases of fracture delayed union or nonunion patients after fracture, the prospective study was randomly divided into Chinese medicine group and normal group, 36 cases in each group, two groups were given bone melon extract, hyperbaric oxygen and extracorporeal shock wave therapy, Chinese medicine group was given with Chican Zhuanggu Decoction for the treatment of 4 weeks. Results： Before treatment, there was no significant difference between the＇two groups of patients with serum ALP, BGP, BMP2, IGF- 1 level （P 〉 0. 05） ; after treat- ment, serum ALP, BGP, BMP2, IGF- I levels of the TCM group patients were lower than the normal group （P 〈 0. 05 ） ; 3 months at the end of treatment, 1 months after treatment, after treatment, the callus formation scores of the TCM group patients were higher than that in the controlled group （ P 〈 0. 05 ） ; 6 months after treatment, the TCM group of patients with fracture healing rate was 88. 89% higher than the conventional group 66. 67% （P 〈 0. 05）, the average fracture healing time of the Chinese medicine group was shorter than the conventional group （P 〈 0. 05 ）. Conclusion ： Using Chishen Zhuanggu Decoction in the treatment of nonunion and delayed union can regulate bone metabolism and promote bone formation, promote healing of fracture.
