摘要
目的探讨宫颈COOK球囊用于足月妊娠期轻度肝内胆汁淤积症(ICP)引产的有效性和安全性。方法选择足月轻度ICP初产妇120例,根据孕妇意愿分为2组,各60例。观察组使用宫颈COOK球囊促宫颈成熟,对照组应用小剂量缩宫素促宫颈成熟。观察两组促宫颈成熟的效果及分娩结局。结果观察组放置cOOK球囊后宫颈Bishop评分较放置前明显提高(P〈0.05),观察组孕妇第一产程时间、总产程时间均明显短于对照组,阴道分娩率明显高于对照组,剖宫产率低于对照组,差异有统计学意义(P〈0.05),两组产后2h出血量、胎儿窘迫发生率及新生儿窒息率比较,差异无统计学意义(P〉0.05)。结论宫COOK球囊应用于妊娠期轻度ICP,引产成功率高,是一种有效又安全的引产方式。
Objective To evaluate the effectiveness and safety of COOK balloon for labor induction in mild intrahepatic cholestasis of pregnancy ( ICP ) . Methods A total 120 cases with mild ICP were divided into two group by the wishes of pregnant women, the experimental group ( n=60 ) used COOK balloon for cervical ripening, and control group ( n=60 ) chose low-dose oxytocin for cervical ripening, the cervical ripening effect and childbirth outcomes of the two groups were observed. Results The first stage of labor time, total labor time of experimental group were shorter than the control group, the vaginal delivery rate was significantly higher and the cesarean section rate was lower in experiment group, compared with the control group, the difference was statistically significant ( P〈0.05 ) , the 2 hours postpartum bleeding, incidence of fetal distress and neonatal asphyxia were no significant difference in the two groups ( P〉0.05 ) . Conclusions COOK balloon is an effective and safe way of induction of labor, can effectively promote cervical ripening for the mild intrahepatic cholestasis of pregnancy.
