颈动脉狭窄患者与其脑梗死复发率的相关性探讨

Discussion of correlation between carotid stenosis and cerebral infarction recurrence rate

目的:探究颈动脉狭窄患者与其脑梗死复发率之间的相关性,指导临床疾病的预防与诊疗。方法:收集200例脑梗死患者的临床资料,并进行多普勒彩超检查,对其颈动脉有无病变以及狭窄程度进行判定。结果:200例患者中,颈动脉狭窄患者的总复发率为23%,腔隙性脑梗死患者的复发率为14.28%(9/63),血栓性脑梗死患者的复发率为27.01%(37/137);颈动脉重度狭窄患者的复发率为50.00%,明显高于轻度9.23%、中度25.30%;溃疡斑患者的复发率为52.78%,明显高于纤维斑块9.52%、软斑30.15%,差异有统计学意义(P<0.05);患者颈动脉狭窄的严重程度与其动脉粥样硬化斑块中溃疡斑、软斑是导致复发的高危因素。结论:患者颈动脉狭窄程度与其脑梗死的复发率存在相关性,要加强对颈动脉狭窄患者的关注与诊断,以更好的预测其脑梗死复发,提升患者生存几率与生存质量,为脑梗死患者的临床防治工作提供有效指导。

Objective： To explore correlation between carotid stenosis and recurrence rate of cerebral infarction, in order to guide the prevention and treatment of the clinical disease. Methods ： 200 patients with cerebral infarction were selected as the objects of the study, the clinical data were collected, and color Doppler ultrasonography was performed to determine the extent of the lesions and the degree of stenosis of carotid artery. Results： Among the 200 cases, the overall recurrence rate of carotid stenosis was 23% , la-cunar cerebral infarction recurrence rate was 14.28% （9/63） , and thrombotic cerebral infarction recurrence rate was 27. 01% （37/ 137）. The recurrence rate of severe carotid stenosis was 50% , significantly higher than mild 9. 23% and moderate 25.30%. The ul-cer recurrence rate was 52. 78% , significantly higher than fibrous plaque 9. 52% and soft plaque 30. 15% （ P〈0. 05）. The severity of carotid stenosis, ulcer plaque in atherosclerotic plaques, and soft plaque were the high risk factors of recurrence. Conclusions： There is a typical correlation between carotid stenosis and cerebral infarction recurrence rate. In order to improve the survival rate and quality of life of the patients with cerebral infarction, the attention and diagnosis of carotid artery stenosis should be enhanced to provide effective guidance for clinical prevention and treatment of cerebral infarction.
