微血管减压术在三叉神经痛治疗中的应用及疗效分析

目的探究微血管减压术在三叉神经痛治疗中的应用及疗效分析。方法选取我院神经科在2013年3月~2015年8月收治的三叉神经痛且选择手术治疗的患者76例,随机分为观察组48例和对照组28例。两组患者均给予常规治疗,在此基础上,对照组予以三叉神经节后射频电凝疗法治疗,观察组则给予微血管减压术治疗。在治疗前及治疗1个月后,观察记录两组患者视觉模拟评分法(VAS),匹茨堡睡眠质量指数量表评分法(PSQI)评分以及1个月后的治疗效果,并用SF-36生存质量量表对两组患者的生存质量进行评估,并以SPSS17.0对所得数据进行对比分析。结果在1个月的治疗后VAS及PSQI评分,观察组与对照组在治疗均明显降低,且两组差异具有统计学意义(P<0.05);1个月后治疗效果评价,观察组明显优于对照组(P<0.05);两组在治疗前的1个月后生活质量对比上,观察组明显优于对照组,差异有明显统计学意义(P<0.05)。结论将微血管减压术运用于三叉神经痛的治疗,不仅能有效降低疼痛程度、提高睡眠质量,还能提高患者术后生活质量,值得临床推广运用。

Objective To explore the application of microvascular decompression in the treatment of trigeminal neuralgia and curative effect analysis. Methods Select 76 cases with tri- geminal neuralgia and the choice of surgical treatment in our hospital neurologist from March 2013 to August 2015 ,divided into observation group and control group randomly, 48 cases in ob- servation group and 28 in control group. Two groups of patients were given conventional treat- ment,on this basis,the control group to trigeminal nerve after radiofrequency electric coagulation therapy,observation group were given microvascular decompression for treatment. Before treat- ment and 1 month after treatment,observation records in the two groups patients visual analogue scale （VAS）,Pittsburgh sleep quality index （PSQI） score and 1 month after treatment effect,and quality of SF--36 scale to evaluate two groups of patients survival quality,and by SPSS17.0 to the obtained data were analyzed. Results After 1 month of treatment VAS and PSQI score were significantly lower after therapy group and control group, and the differences between the two groups have statistical significance （P〈0.05）;1 month after treatment effect evaluation,the ob- servation group was better than control group （P〈0.05）;The two groups before treatment on the quality of life after 1 months of contrast, the observation group was better than control group, difference has the obvious statistical significance （P〈0.05）. Conclusion Application of micro- vascular decompression in the treatment of trigeminal neuralgia, not only can effectively reduce the degree of pain, improve the quality of sleep, also can improve the patients quality of life, worth clinical promotion.
