痰热清对慢性阻塞性肺疾病急性加重期患者的疗效及对PaCO2、FEV1%水平的影响

The effect of Tanreqing Injection on chronic obstructive pulmonary disease in acute exacerbation and on levels of PaCO2 and FEV1%

目的:探讨痰热清对慢性阻塞性肺疾病的治疗效果,及对患者血气和肺功能指标的影响。方法:回顾性分析2011年4月~2016年8月在我院接受治疗的157例慢性阻塞性肺疾病患者的临床资料,根据其治疗方式分为对照组和治疗组,对照组给予西药治疗,治疗组在此基础上给予痰热清。观察两组患者治疗前后血气、肺功能和血清炎症细胞因子水平。结果:1治疗前两组血气指标无差别,治疗后治疗组血红蛋白(Hb)、血氧饱和度(SpO2)水平明显高于对照组,动脉血二氧化碳分压(PaCO2)水平明显低于对照组。2治疗前两组患者的肺功能指标水平无明显差异。治疗后治疗组的6分钟步行试验(6MWT)、用力肺活量(FVC)、一秒用力呼气容积(FEV1)、FEV1/FVC、最大呼气中期流量(MMEF)和呼气流量峰值(PEF)水平明显高于对照组。3治疗前两组患者血清炎症细胞因子水平无差别。治疗后治疗组的白细胞介素18(IL-18)、白细胞介素6(IL-6)和超敏C-反应蛋白(hs-CRP)水平明显低于对照组。结论:痰热清对慢性阻塞性肺疾病急性加重期患者有较好的治疗效果,可明显改善患者血气和肺功能指标,具有较好的临床应用价值。

Objective： To investigate the therapeutic effect of Tanreqing Injection on chronic obstructive pulmonary disease,and their effects on blood gas and lung function index.Methods： The clinical data of patients with chronic obstructive pulmonary disease who were treated in our hospital from April 2011 to August 2016 were analyzed.According to the treatment methods patients were divided into western medicine treatment group（ control group） and Tanreqing injection combined with western medicine treatment group（ observation group）.After 4 weeks treatment,the blood gas,lung function and serum inflammatory cytokine levels were observed in the two groups before and after treatment.Results： 1After treatment,Hb,levels of SpO2 and Pa CO2 of observation group were better than that of control group（ P 〈 0.05）.2After treatment,levels of 6MWT,FVC,FEV1,FEV1/FVC,MMEF and PEF of observation group were better than that of control group（ all P 〈 0.05）.3After treatment,levels of IL-18,IL-6 and hs-CRP of observation group were lower than that of control group（ all P 〈 0.05）.Conclusion： The effect of Tanreqing injection combined with western medicine was better,and it can significantly improve the blood gas and pulmonary function.
