通络生乳方治疗剖宫产后缺乳临床效果观察

Clinical observation on the treatment of post-cesarean hypogalactia by Tongluoshengru prescription

目的:评价通络生乳方治疗剖宫产术后产妇缺乳的临床疗效及其安全性。方法:将202例剖宫产术后缺乳产妇随机分为治疗组(n=102),口服给予通络生乳方连续7天,每日3次,每次50 ml(每次相当于50 g生药);对照组(n=100)不服用任何催乳药物。治疗前后进行常规体检,观察乳房胀度,测量血清泌乳素、泌乳量和乳汁中蛋白质含量,评价疗效。结果:观察期间未见到明显不良反应。两组产妇在治疗前的血清泌乳素、乳房胀度和泌乳量比较差异均无统计学意义。治疗组产妇服用通络生乳方7天后,血清泌乳素水平达到(369.2±87.7)μg/L,乳房胀度显著增加,总积分均值达(7.6±2.6),泌乳量平均增加了(24.0±14.7)ml,与自身试验前及对照组比较,差异均有统计学意义。治疗7天后,总有效率治疗组达90.2%,与对照组的44.0%比较,差异有统计学意义,而两组产妇乳汁中蛋白质含量未见明显差别。结论:通络生乳方对剖宫产术后缺乳产妇安全有效。

Objective： To evaluate the clinical therapeutic effects and safty of Tongluoshengru prescription on patients suffered post-cesarean hy- pogalactia. Methods： 202 patients suffered post-cesarean hypogalactia were randomly divided into an therapeutic group （n = 102） and a con- trol group（ n = 100）. Volunteers in the therapeutic group were given orally 50 ml（ equivalent to 50 g crude drug） Tongluoshengru prescription t. i. d while those in the control group were given none. The course was 7 days. All patients were taken routine physical examinations, breast swelling grade, serum prolactin levels and milk secretion volume were measured before and after the experiment. And contents of milk protein were detected at the end of the experiment. Results： No adverse effects were observed during the whole treatment. The indicators of the two groups which included breast swelling grade, serum prolaetin levels and milk secretion volume had no differences before the treatment（ P 〉 0. 05 ）. After the patients in the therapeutic group had consumed Tongluoshengru prescription for 7 days, the mean of serum prolactin levels reached （ 369.2 ± 87.7 ） μg/L, breast swelling grade significantly increased and the mean of total integral was （ 7.6 ± 2.6 ）. As well as milk secretion volume increased evenly （ 24.0 ± 14.7 ） ml. All of above had significantly differences which compared with themselves before the treatment and the control group（ P 〈0.01 ）. At the end of the treatment, the total effective rate in the therapeutic group was 90.2%, while that in the control group was 44.0% , which had significantly differences （P 〈 0.01 ）. Meanwhile, the contents of milk protein between the two groups did not show significant difference （ P 〉 0.05 ）. Conclusion： Tongluoshengru prescription is safe and effective for patients suffered post-cesarean hypogalactia.
