3HRZL/9HR方案治疗结核性脑膜炎的临床疗效

Clinical effect of 3HRZL/9HR program on tuberculous meningitis

目的探讨3HRZL/9HR方案治疗结核性脑膜炎的临床疗效。方法选取东台市人民医院2011—2015年收治的结核性脑膜炎患者35例,根据随机数字表法分为改良组(n=18)和对照组(n=17)。患者入院后均给予常规对症支持治疗。对照组患者采用传统3HRZS/9HR治疗方案,改良组患者采用3HRZL/9HR治疗方案,两组患者均持续治疗3个月。比较两组患者的临床疗效及治疗前后脑脊液相关指标(细胞计数、蛋白定量、氯化物和葡萄糖定量)。观察两组患者毒副反应发生情况。结果改良组患者临床疗效优于对照组(P<0.05)。治疗前,两组患者细胞计数、蛋白定量、氯化物及葡萄糖定量比较,差异无统计学意义(P>0.05);治疗后,改良组患者细胞计数、蛋白定量低于对照组,氯化物、葡萄糖定量高于对照组(P<0.05)。两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论采用3HRZL/9HR方案治疗结核性脑膜炎的临床疗效确切,可明显改善脑脊液相关指标,促进疾病康复。

Objective To evaluate the clinical efficacy of 3HRZL/9HR program on tuberculous meningitis.Methods A total of 35 cases of tuberculous meningitis patients were selected from 2011 to 2015 year in Dongtai People＆#39;s Hospital,which were divided into modified group（n=18） and control group（n=17） according to random number table method.All patients were given routine symptomatic support therapy,control group were given traditional 3HRZS/9HR treatment program,modified group were given 3HRZL/9HR treatment program.The clinical effect and cerebrospinal fluid related indexes（cell counting,protein content,chloride, glucose quantitative） before and after treatment were compared between the two groups.The adverse reactions were observed between the two groups.Results The clinical effect of modified group was better than control group（P〈0.05）.Before treatment,no statistically significant differences of cell counting,protein content,chloride or glucose quantitative was found between the two groups（P〉0.05）;after treatment,the modified group of cell counting,protein content were lower than control group,chloride, glucose quantitative were higher than control group（P〈0.05）.There was no statistically significant of adverse reactions rate between the two groups（P〉0.05）.Conclusion 3HRZL/9HR program has an obviously clinical effect on tuberculous meningitis,it can effectively improve the cerebrospinal fluid related indicators,and promote the disease recovery.
