综合干预治疗对重型β-地中海贫血青春期女性生长发育的效果观察 被引量:8

摘要 目的:探讨综合干预治疗对重型β-地中海贫血(β-TM)的青春期女性患者性腺发育功能的影响。方法:收集本院确诊的重型β-TM青春期女性患者45例,并收集同期本院体检正常女性15例为对照组。测定两组对象身高、体重,采静脉血检测输血前后血红蛋白(Hb)、促黄体生成素(LH)和促卵泡生成素(FHS)水平、雌二醇(E_2),超声检测性腺发育情况并进行对比分析。结果:治疗前各血清蛋白(SF)浓度组重型β-TM患者的身高、体重、Hb、LH、FSH、E_2、子宫容积、左侧卵巢容积和右侧卵巢容积均低于对照组(P<0.05),且随着患者SF的增加呈现降低趋势。经过规范化治疗后各SF浓度组重型β-TM患者的上述检测指标均有所增加并趋近正常水平,治疗前后比较有差异(P<0.05),治疗后组间比较无差异(P>0.05)。结论:综合规范的干预治疗可恢复重型β-TM的青春期女性的正常身体发育、性腺发育和性激素水平,临床应加以重视和规范治疗。 Objective: To explore the effectiveness of comprehensive intervention on gonad development and its function of adolescent female patients with severe β-thalassemia. Methods: 45 adolescent female patients with severeβ-thalasse- mia were in observation group, and 15 normal female adolescent were in control group at the same time. Hemoglobin (Hb), Luteinizing hormone (LH) and follicle stimulating hormone (FHS), estradiol (E2) of all patients were detected by laboratory before and after treatment with transfusion. And gonadal development situation was measured by ultrasonic detection. Results: Before treatment, the height, weight, HB, LH, FHS, E2 , uterine volume, ovarian volume of patients with β-thalassemia were all significant lower than those of female adolescent in control group(P〈0.05), and the numerical value of indicators above were decreased with the increase of serum ferritin concentration. After treatment, the indicators above increased and reached to normal. There were significant difference in height, weight, HB, LH, FHS, E2, uterine volume, and ovarian volume of patients with β-thalassemia before and after treatment(P 〈0.05). After treatment, there were no significant differences in above indicators between observation group and con- trol group (P〉0.05). Conclusion: Comprehensive standard intervention can improve normal body development, go- nadal development and sex hormone levels of adolescent female patients with severe β-thalassemia, which is worthy to clinical promotion.
