丙戊酸、左乙拉西坦单药治疗对学龄前期及学龄期癫痫患儿骨代谢的影响 被引量:12

摘要 目的探讨丙戊酸(VPA)、左乙拉西坦(LEV)单药治疗对学龄前期及学龄期癫痫患儿骨代谢的影响。方法选择2015年7月至2016年7月在南方医科大学附属沧州市人民医院儿科门诊采用单药治疗的学龄前期癫痫患儿39例(采用VPA治疗20例、采用LEV治疗19例)及学龄期癫痫患儿40例(采用VPA治疗21例、采用LEV治疗19例)作为观察组;同时选择体检正常健康儿童41名(学龄前期21名、学龄期20名)作为健康对照组。分别记录健康对照组儿童血清钙(Ca^2+)、磷(P^3-)、碱性磷酸酶(ALP)、25-羟基维生素D[25一(OH)D]水平及各观察组患儿治疗前、治疗3个月后、治疗6个月后的血清Ca^2+、P^3-、ALP、25^-(OH)D水平,并进行比较。结果采用LEV单药治疗的学龄前期和学龄期患儿治疗3个月、6个月后血清Ca^2+、P^3-、ALP、25-(OH)D水平与治疗前比较差异均无统计学意义(均P〉0.05)。采用VPA单药治疗的学龄前期患儿治疗3个月、6个月后的血清ca“、P”、ALP、25-(OH)D水平与治疗前比较差异均无统计学意义(均P〉0.05);采用VPA单药治疗3个月后的学龄期患儿血清Ca^2+、P^3-、ALP、25-(OH)D水平与治疗前比较差异均无统计学意义(均P〉0.05),而治疗6个月后其血清ALP[(250±32)U/L]较治疗前[(220±34)U/L]升高,差异有统计学意义(P〈0.05),血清Ca^2+、P^3-、25-(OH)D水平无显著变化(均P〉0.05)。结论LEV单药治疗对学龄前期及学龄期患儿骨代谢影响不大;VPA单药治疗6个月内即对学龄期儿童ALP产生影响。因此应注意服用VPA的学龄期患儿ALP的早期监测,对其具体机制还需进一步观察和研究。
