摘要 目的:探究羟乙基淀粉(HES)对急性缺血性脑卒中患者肾功能的影响。方法:选择急性缺血性脑卒中患者62例作为试验组,在常规治疗的基础上,给予HES130/0.4氯化钠500 mL静脉滴注,1次/d,疗程14 d。55例急性缺血性脑卒中患者作为对照组,给予常规治疗。收集入院时、治疗第7天、治疗第14天、治疗后第1个月及治疗后第3个月时的神经功能缺损程度(NIHSS评分)、日常生活能力指数(BI)及肾功能变化情况。结果:2组患者入院时的传统危险因素差异无统计学意义(P>0.05)。治疗后试验组患者的NIHSS评分、BI均较对照组明显改善(P<0.05);治疗期间肌酐、尿素氮、估计肾小球滤过率(e GFR)均有恶化趋势,但均逐渐恢复。结论:HES治疗急性缺血性脑卒中患者后,其生活质量可改善明显,肾功能在治疗后均逐渐恢复。 Aim: To explore the effect of hydroxyethyl starch( HES) 130/0. 4 sodium chloride injection on acute ischemic stroke patients’ renal function. Methods: A total of 62 hospitalized patients with acute ischemic stroke were chosen as trail group,and were given 500 mL HES 130/0. 4 sodium chloride injection once per day for 14 days based on conventional treatment. Another 55 hospitalized patients with acute ischemic stroke( control group) were given conventional treatment. The NIHSS,daily life index and changes of renal function on the 7th day,the 14 th day after treatment as well as after1 month and 3 months were collected. Results: There were no differences in the traditional risk factors between the 2groups on admission; all patients’ NIHSS,daily life index after treatment improved significantly than on admission( P 〈0. 05). The creatinine,urea nitrogen and estimated glomerular filtration rate had a degradation trend during the treatment in the trial group( P 〈 0. 05),however,after treatment all indexes improved. Conclusion: HES 130/0. 4 could cause the acute ischemic stroke patients’ renal function injury,but the neurological deficits and the daily life index improved significantly after the treatment,and the situation of kidney injury treatment gets better.
