摘要
目的探讨HBV-DNA、HBs Ag及凝血功能检测在老年慢性乙型肝炎中的应用及临床意义。方法选取2013年1月~2016年1月期间的我院收治的老年慢性乙肝患者110例(年龄均>60岁),分为老年慢性重型乙肝组(A组)为46例,老年慢性乙肝组(B组)64例,作为观察组;同时随机抽选老年组人群110例作为对照组,并称之为老年健康组(C组,年龄均>60岁)。分析不同年龄阶层老年慢性乙肝患者的HBs Ag定量,探讨老年慢性乙肝患者的HBV-DNA与HBs Ag的相关性,同时分析在老年慢性乙肝患者不同HBV-DNA定量分组中凝血功能的特点。结果在老年慢性乙肝患者中,其HBs Ag和患病时间长度呈现负相关的关系。HBV-DNA数值越高,其HBs Ag的水平也越高。HBV-DNA含量越高,PT、APTT、TT的数值也越高,相反,FIB的数值却越低。结论 HBV-DNA与HBs Ag、凝血功能等多方面检测是能够比较客观和科学地反映老年慢性乙型肝炎患者的病情,在老年慢性乙型肝炎患者的诊治和疗效评估中具有积极的临床意义。
Objective To investigate the application and the clinical significance of the testing about HBV-DNA ,HBsAg and the coagulation of elderly patients with chronic hepatitis B.Metbods Confirmed elderly patients with chronic hepatitis B accepted in our hospital from January 2013 to January 2016 (total 110cases , age over 60) were randomly selected and divided into elderly patients with severe hepatitis B group (46 cases,named A) and elderly patients with chronic hepatitis B (64 cases,named B).and the same time elderly health people were elected and called C as the control group ( age over 60 ). Quantitative HBV-DNA of elderly patients with chronic hepatitis B in different age classes were analyzed. And the correlation of HBV-DNA ,HBsAg was investigated .In addition the characteristics of the coagulation Were dissused among the different groups of quantitative HBV-DNA about elderly patients with chronic hepatitis B. Results The relationship between HBsAg and the duration of illness was negatively correlated among elderly patients with chronic hepatitis B. The HBV-DNA value got higher, the HBsAg level was higher. The HBV-DNA contents became higher, PT,APTT,TT were higher.On the contranry the value of FIB got lower. ConclusionsThe the testing about HBV-DNA ,HBsAg and the coagulation were able to compare objectively and scientific.ally reflect the condition of elderly patients with chronic hepatitis B, it had positive clinical significance in the diagnosis and treatment of elderly patients with chronic hepatitis B.
