肺炎克雷伯菌耐药与抗菌药物使用强度的相关性分析 被引量:24

摘要 目的分析医院2007-2016年住院患者肺炎克雷伯菌耐药率和抗菌药物使用量之间的关系。方法采用回顾性调查方法,统计2007-2016年住院患者抗菌药物使用强度(AUD),采集2008-2016年全年医院住院患者肺炎克雷伯菌的耐药率,并对数据采用Spearman相关法进行分析。结果肺炎克雷伯菌的耐药率逐年上升,同年的美罗培南使用强度(AUD)和耐碳青霉烯类肺炎克雷伯菌(CRKP)检出率有显著相关(P<0.05);碳青霉烯类总AUD与次年CRKP检出率有显著相关(P<0.05),亚胺培南AUD与次年CRKP检出率尽管无统计学相关性,但有相关趋势。结论 CRKP检出率与碳青霉烯类药物用量存在一定相关性,应加强碳青霉烯类药物的合理使用。 OBJECTIVE To investigate the correlation between the resistant rate of Klebsiella pneunioniae (KPN) and antimicrobials consumption.METHODS The resistant rates of KPN (from 2008 to 2016) and the antibiotics use density (AUD) (from 2007 to 2016) of inpatients were retrospectively analyzed,and the linear regression between them was conducted by Spearman correlation method.RESULTS The resistant rate of KPN increased gradually.The rates of CRKP were significantly positive correlated to the AUD of meropenem of the same year(P 〈0.05).The rates of CRKP were significantly positive correlated to the total AUD of carbapenem of the last year (P 〈0.05).The rates of CRKP were not significantly positive correlated to the AUD of imipenem,but there had tendency of correlation.CONCLUSION The correlation is found between carbapenem consumption and the rate of CRKP.It is necessary to strengthen the management of the rational use of carbapenem.
