摘要
目的：探讨血浆D-二聚体（D-D）和糖类抗原724（CA724）水平在胃癌患者手术前后的变化及临床意义.方法：选取经病理证实的胃癌患者60例作为胃癌组,另选50例胃良性病变患者作为胃良性病变组及健康体检者50例作为健康对照组,观察各组血浆D-D和CA724水平,并进行比较.结果：治疗前胃癌组、 胃良性病变组及健康对照组血浆D-D和CA724水平依次降低,差异具有统计学意义（P〈0.05）;且血浆D-D和CA724水平与胃癌临床分期相关,分期越晚,二者水平越高,其差异具有统计学意义（P〈0.05）;治疗后胃癌组血浆D-D和CA724水平明显下降,但仍明显高于健康对照组,差异具有统计学意义（P〈0.05）,与胃良性病变组相比,差异无统计学意义（P〉0.05）;胃癌组血浆D-D和CA724联合检测显著高于单项检测,其差异具有统计学意义（P〈0.05）.结论：胃癌患者手术前后血浆D-D和CA724水平变化显著,且与临床分期呈正相关,能较好反映胃癌患者疾病状况,两者联合检测能提高胃癌患者诊断敏感性和准确性,值得临床推广.
Objective： To explore the changes of plasma D-D and Carbohydrate antigen 724 （CA724） in patients with gastric cancer before and after surgery and its clinical significance. Methods： 60 patients with gastric cancer, 50 patients with benign gastric disease and 50 healthy controls were chosen in our study. The levels of D-D and CA724 in plasma in different groups were tested and compared. Results： Levels of plasma D-D and CA724 in patients with gastric cancer were significantly higher than those in group of benign gastric disease and healthy control group （P〈0.05） . Plasma D-D and CA724 levels were corelated with clinicall stages of gastric carcinoma, levels of plasma D-D and CA724 were higher with the progression of tumors （P〈0.05） . After surgical treatment, D-D and CA724 levels in gastric cancer group were significantly higher than those in healthy control group （P〈0.05） . There is no significantly difference when we compared D-D and CA724 levels in benign gastric disease group with those in healthy control group （P〉0.05） .When we conbine D-D with CA724 （P〈0.05） . Conclusion： Plasma D-D and CA724 levels were significantly changed in gastric cancer patients before and after surgery, and were corelated with clinical stages. Thus, they can reflect the severity of the disease, and the combined detection of D-D and CA724 can improve the sensitivity and accuracy of diagnosis of gastric cancer, which is worthy ofrecornmendatior~
