CT血管造影在缺血性脑卒中颈内动脉形态评估中的效果

Analysis of the effect of CT angiography in the evaluation of carotid artery in ischemic stroke

目的研究CT血管造影在缺血性脑卒中颈内动脉形态评估中的效果分析。方法选取我院2015年1月到2015年12月期间接诊的40例缺血性脑卒中患者作为研究对象,使所有患者均进行颈部血管彩超检查、CT血管造影及数字减影血管造影(DSA)检查,以DAS检查结果作为基准,检测并记录两种方法的颈内动脉狭窄以及颈内动脉斑块形态情况等。结果根据DAS检查结果,40例患者中共发现83支颈动脉狭窄,若以DAS检查结果为基准,则采用CT血管造影的方法进行检查的检出率为77.1%,采用颈部血管彩超检查的检出率为73.5%,差异无统计学意义(P>0.05)。采用CT血管造影的检查方法对40例患者进行检查,共发现94个斑块,其中光滑斑块40个,不规则斑块19个,溃疡斑块35个,溃疡斑块检出率为37.2%,而采用颈部血管彩超进行检查时,共发现79个斑块,其中光滑斑块38个,不规则斑块22个,溃疡斑块19个,溃疡斑块检出率24.1%,差异具有统计学意义(P<0.05)。结论采用CT血管造影技术进行检查,可以更加精确地对患者的颈内动脉狭窄以及形态进行评估,并可以更加准确的判断患者的斑块性质,为患者的临床诊断提供更多的参考信息。值得推广。

Objective： To study the effect of CT angiography in the evaluation of carotid artery in patients with ischemie stroke. Methods： Forty cases of patients with ischemie stroke in our hospital from January 2015 to December 2015 were as the research subjects, all patients had neck vascular ultrasound examination and CT angiography and digital subtraction angiography （DSA） examination. Based on the examination results of DAS, carotid artery stenosis and coronary artery plaque forms were tested and recorded. According to the results of DAS examination results, 83 cases of carotid artery stenosis were found in 40 patients. With DAS results in the detection method being the benchmark, CT angiography examination rate was 77.1%, the detection of neck vascular ultrasound examination rate was 73.5%, the difference was not statistically significant （ P 〉 0. 05 ）. The examination method of CT angiography was performed in 40 patients, 94 were found in plaque, with 40 smooth patches, 19 irregular plaque, ulcer plaque 35, ulcer plaque detection rate was 37.2%. In the neck vascular ultrasound examination, 79 plaques were found, with smooth 38 plaques, 22 irregular plaque and 19 ulcer plaque, and ulcer plaque detection rate was 24.1%. The difference was statistically significant （P 〈 0.05 ）. Conclusion： CT angiogral：hy examination can evaluate the internal carotid artery in patients with stenosis and plaque morphology more accurately, and can help evaluate the patients＇plaque more accurately to provide more information for clinical diagnosis of patients, which is worth promoting.
