苄星青霉素治疗以湿疹皮炎为表现的梅毒研究

Study of eczema manifestations of patients with syphilis and its treatment

目的:研究梅毒患者的湿疹皮炎表现及其治疗。方法:选取南京454医院皮肤科2012年5月至2016年5月收治的以湿疹皮炎表现的梅毒患者40例,确诊后给予所有患者肌肉注射240万U苄星青霉素,1次/周,3周为1个疗程,然后对其瘙痒消失时间、疹消退时间及复发情况进行分析,并对其治疗前后的TRUST滴度变化情况进行统计分析。结果:40例患者治疗后瘙痒消失时间3~12d,平均(7.4±1.3)d;皮疹消退时间2~3周,平均(2.5±0.5)周。对患者进行半年的随访,所有患者均无复发;治疗后的TRUST滴度1∶8、1∶16的比例为50.0%、37.5%,均显著高于治疗前的37.5%、25.0%(P<0.05),1∶32、1∶64、1∶128、1∶256比例分别为7.5%、5.0%、0、0,均显著低于治疗前的17.5%、12.5%、5.0%、2.5%(P<0.05)。结论:以湿疹皮炎为表现的梅毒患者苄星青霉素治疗能够有效缩短患者的瘙痒消失时间和皮疹消退时间,降低患者的TRUST滴度,且不会引发患者复发,效果显著。

Objectives： To study the performance of eczema patients with syphilis and its treatment.Methods： 40 patients with eczema manifestations of syphilis who were treated in the Department of Dermatology of Nanjing 454 hospital from May 2012 to May 2016 were selected. All patients were given 2. 4 million U intramuscular benzathine penicillin after diagnosis,once a week and 3 weeks as a course of treatment. The itching disappear time,rash subsided time and recurrence were analyzed and the changes in TRUST titers between before and after treatment were statistically analyzed. Results： The itching disappeared time after treatment of the 40 patients was 3-12 d,an average of（7. 4 ± 1. 3） d. The rash subsided time was 2-3 weeks,an average of（2. 5 ± 0. 5） weeks. Patients were followed up for 6 months,without recurrence. The ratios of TRUST titers after treatment of 1 ∶ 8（＋）,1 ∶ 16,（＋） 50. 0%（20/40）,37. 5%（15/40） were significantly higher than those before treatment,i. e. 37. 5%（15/40）,25. 0%（10/40）（P〈0. 05）; the ratios of 1 ∶ 32（＋）,1 ∶ 64（＋）,1 ∶ 128（＋） 1 ∶ 256（＋）7. 5%（3/40）,5. 0%（2/40）,0,0 were significantly lower than those before treatment,i. e. 17. 5%（7/40）,12. 5%（5/40）,5. 0 %（2/40）,2. 5%（1/40）（P〈0. 05）. Conclusion： Benzathine penicillin can shorten the itching disappeared time and rash subsided time,reduce the TRUST titers,without recurrence.
