综合外剥内治(扎)法治疗环状混合痔临床观察

Clinical observation of comprehensive external dissection and internal ligation in the treatment of circular mixed hemorrhoids

目的考察综合外剥内治(扎)法治疗环状混合痔的疗效。方法 248例环状混合痔患者随机分为治疗组(126例)和对照组(122例)。治疗组采用综合外剥内治(扎)法治疗,对照组采用传统的外剥内扎法,观察两组患者的住院时间、手术伤口完全愈合时间、手术伤口水肿、疼痛等指标。结果治疗组住院时间为(6.30±1.46)d,少于对照组的(9.40±1.57)d,差异有统计学意义(P<0.05)。治疗组手术伤口完全愈合时间为(13.46±1.47)d,少于对照组的(16.80±1.86)d,差异有统计学意义(P<0.05)。治疗组手术伤口水肿、尿潴留、创口疼痛率明显低于对照组,差异有统计学意义(P<0.05)。两组患者的治愈率、大出血情况比较,差异无统计学意义(P>0.05)。结论综合外剥内治(扎)法治疗环状混合痔可缩短患者完全愈合时间、减少患者痛苦、较好保护肛门功能。

Objective To observe the curative effect of comprehensive external dissection and internal ligation in the treatment of circular mixed hemorrhoids. Methods A total of 248 circular mixed hemorrhoids patients were randomly divided into treatment group （126 cases） and control group （122 cases）. The treatment group received comprehensive external dissection and internal ligation for treatment, and the control group received traditional external dissection and internal ligation. Observation were made on hospital stay time, surgical wound complete healing time, surgical wound edema in two groups. Results The treatment group had less hospital stay time as （6.30 ＋ 1.46） d than （9.40 ＋ 1.57） d in the control group, and the difference had statistical significance （P〈0.05）. The treatment group had less healing time as （13.46 ± 1.47） d than （16.80± 1.86） d in the control group, and the difference had statistical significance （P〈0.05）. The treatment group had obviously lower incidence of surgical wound edema, uroschesis and wound pain than the control group, and their difference had statistical significance （P〈0.05）. Both groups had no statistically significant difference in cure rate and massive hemorrhage condition cases （P〉0.05）. Conclusion Comprehensive external dissection and internal ligation can shorten complete healing time, reduce pain of patients and protect anal function.
