主动性肛门直肠异物诊治及可能病因分析

Diagnosis and treatment of active anorectal foreign body and analysis of possible etiology

主动性肛门直肠异物病因不明,人们、甚至医生往往以性变态、精神异常而毫无依据地搪塞之,给患者及患者家属造成极大伤害。2005~2015年我院急诊科接诊主动性肛门直肠异物5例。现报告如下。1病例资料5例患者均为男性,其中1例为62岁,农民,配偶健在。将农村抽旱烟用的约20cm烟袋完全插入肛门直肠。2h后就诊时,肛门直肠指诊可触诊到活动烟袋锅,顺手将其勾出,患者恢复好。

The clinical data of 5 cases with active rectal foreign bodies treated by emergency department and general surgery department in our hospital from 2005 to 2015 were analyzed retrospectively. According to their specific situation,5 cases with active rectal foreign bodies had taken different treatment measures. One patient was discharged without abdominal pain or bloody stool after the foreign body was removed through the anus in the e- mergency department. The foreign bodies in two hospitalized patients in general surgery department were removed through the anus in epidural anesthesia. One patient underwent exploratory laparotomy, and one patient died who had chosen medical conservative treatment and refused surgical treatment. Active anal foreign body is rare and its specific etiology is not clear. Diagnosis and treatment should be made differently according to different kinds of specific clinical manifestations and foreign bodies. The dislocation of the erogenous zone and disorder of pelvic floor nerve conduction may be the pathophysiological basis of active anal foreign body.
