摘要
目的探讨甘氨双唑钠联合鸦胆子油乳介入治疗原发性肝癌的疗效及对血清中可溶性B7-H4的影响。方法将110例原发性肝癌患随机分为对照组和治疗组各55例。对照组采取缓慢注入比柔比星、顺铂、氟尿嘧啶以及用比柔比星与碘化油混悬剂栓塞肿瘤血管。治疗组缓慢注入鸦胆子油,用鸦胆子油与碘化油混悬剂栓塞肿瘤血管;同时给予甘氨双唑钠静脉滴注。比较两组临床受益反应率、不良反应及疾病缓解率。监测两组血清可溶性B7-H4水平。结果治疗组患者临床受益率明显高于对照组(P<0.01,P<0.05)。治疗组不良反应发生率为37.08%,对照组为54.01%,治疗组明显低于对照组(χ2=6.39,P<0.05)。治疗组近期总缓解率为90.91%,对照组为72.73%,治疗组高于对照组(P<0.01)。治疗后2周和1个月,治疗组患者血清中可溶性B7-H4水平显著低于对照组同期,有显著性差异(P<0.01)。结论甘氨双唑钠联合鸦胆子油乳介入治疗原发性肝癌的疗效显著,下调血清中可溶性B7-H4水平可能与其疗效有关。
Objective To study the efficacy of sodium glycididazole plus brucea javanica oil emulsion in treating primary liver cancer, and the influence on serum level of soluble 137-H4. Methods 110 cases with primary liver cancer were randomly di- vided into the control group and the treatment group,55 cases in each group. The control group was treated with amrubicin hydro-chloride, cisplatinum,fluorouracil, and suspensions of amrubicin hydrochloride and iodinated oil bolting tumor blood vessels. The treatment group was treated with brucea javanica oil emulsion, suspensions of brucea javanica oil emulsion and iodinated oil blo- ting tumor blood vessels, and sodium glycididazole with intravenous drip. The clinical benefit rates, the incidence of toxic side effects ,and disease remission rate were compared between the 2 groups. Serum level of soluble BT-H4 was detected in both groups. Results Clinical benefit rate of the treatment group was higher than that of the control group ( P 〈 0.01, P 〈 0.05 ). The incidence of toxic side effects in the treatment group(37.08% ) was less than that of the control group(54.01% )(X^2 = 6.39,P 〈0.05 ). The total remission rate of the treatment group was 90.91%, which was superior to the control group,72.73% ( P 〈 0.01 ). Serum level of soluble B7-H4 of the treatment group was higher than the control group 2 weeks and 1 month after treatment with statistic difference( P 〈0.01 ). Conclusion Efficacy of sodium glycididazole plus brueea javanica oil emulsion in treating primary liver cancer is significant, and may be related to down-regulation of serum level of soluble B7-H4.
