体检相关因子的合理选配对恶性肿瘤早期防控的意义

目的回顾性分析体检相关因子的合理选配对恶性肿瘤的早期防控意义,探讨如何提高健康体检人群中的恶性肿瘤检出率。方法以近3年健康体检上海电力职工89630例中发现并病理确诊的190例恶性肿瘤患者为研究对象。观察各种肿瘤检出率以及相关因子表达。结果病理确诊恶性肿瘤的检出率为0.212%(190例)。前4位恶性肿瘤依次为肺癌0.125%(112例)、结直肠癌0.016%(14例)、肝癌0.011%(10例)和乳腺癌0.010%(9例)。各种肿瘤患者的各项检测指标均明显高于健康对照组,P<0.05,其中AFP在肝癌患者中表达水平最高,CA15-3在乳腺癌患者中表达水平最高,CEA在肠癌患者中表达水平最高,P<0.05,各种肿瘤标志物联合检测的阳性检出率均明显高于单独检测,P<0.05。结论健康体检是早期发现恶性肿瘤的重要途径,而不同肿瘤标志物对恶性肿瘤的检出作用各有特点,4项恶性肿瘤指标联合检测的阳性检出率均明显高于各项指标单独检测。体检相关因子对恶性肿瘤的早期防控有重要的意义。

Objective To retrospectively analyze the prevention and control of reasonable selection of malignant tumor related factor, and discusses how to improve the healthy check-up of the crowd malignant tumor detection rate. Methods Selected 190 cases of malignant tumor patients who received pathologic diagnosis out of 89630 shanghai electric power worker who received health physical examination for nearly three years as the research object. Observe all sorts of cancer detection rate and related factors. Results The pathological diagnosis of malignant tumor detection rate was O. 212% （ 190 cases）, the first four malignant tumors were lung cancer by O. 125% （ 112 cases） ,colorectal cancer by 0.016% （ 14 cases） ,liver cancer by 0. 016% （ 10 cases） and breast cancer by O. 010% （9 cases）. Detection indicators of groups of tumor patients were significantly higher than that of healthy control group, P 〈 0.05, the expression level of AFP was highest in patients with liver cancer, CA15-3 was highest expression in patients with breast cancer, CEA was highest expression in patients with colorectal cancer, P 〈 0.05, positive detection rate of joint detection for the malignant tumor were significantly higher than that of single test,P 〈0. 05 ）. Conclusion Physical examination is an important approach for early detection of malignant tumor, while low-dose lung CT, ultrasound, detection of tumor markers for malignant tumor role each have its Characteristics, positive detection rate of using joint detection of 4 measures of malignant tumor are significantly higher than that of single detection, the indicators can take examination and help for the clinical differential diagnosis of benign and malignant tumor and for the prognosis.
