针刺经验对针灸即刻降压效果影响的临床研究

Clinical Research on the Influence of Acupuncture Experience on Immediate Antihypertensive Effect of Acupuncture

目的观察具有不同针刺经验医师给予患者相同针刺降压治疗时是否产生不同的临床疗效。方法采用前瞻、随机、双盲、平行对照设计,纳入病例被随机分为A、B两组进行统一方案的针灸治疗,给A组患者治疗的医师经过治疗方案的培训并具备最基本的针灸治疗的经验,给B组患者治疗的医师经过治疗方案的培训且具有5年以上针灸治疗经验。针刺的穴位为双侧合谷(LI4)、太冲(LR3)、阴郄(HT6)穴。记录针刺前、针刺30 min后、针刺120 min后血压数值、单项症状和中医证候积分情况。结果高血压患者接受统一制订的针刺降压方案取得较好的临床结果,针刺30 min和针刺120 min时记录的收缩压、舒张压和中医证候积分均较治疗前有明显下降(P<0.05)。两组患者比较,血压变化、单项症状和中医证候积分均无显著差异。结论针对高血压某一证候制订统一的针刺降压方案是可行的,患者的专注度、患者对针感的反馈、合理的选穴对于患者的针刺效果可能起到决定性的作用。

Objective： To observe whether the clinical effects are the same or not,when doctors with different acupuncture experience give patients the same acupuncture treatment. Methods： The patients were selected by prospective,randomized,double-blind,parallel controlled design,and randomly divided into A group and B group. All patients were treated with the same acupuncture design. The doctors treating A group were trained with the treatment plan and had the basic experience of acupuncture treatment,the doctors treating B group with 5 years＆#39; experience of acupuncture treatment. The acupoints were Hegu （LI4）,Taichong （LR3） and Yinxi （HT6）. Blood pressure value,single symptom and TCM syndrome scores were recorded before acupuncture,30 minutes after acupuncture and 120 minutes after acupuncture. Results： Hypertensive patients accepted the unified formulation of the antihypertensive program of acupuncture,which make a good clinical effect. Systolic blood pressure,dias-tolic blood pressure and TCM syndrome scores of 30 minutes after acupuncture and 120 minutes after acupunc-ture were significantly lower than those before treatment （P〈0.05）. There was no significant difference in blood pressure change,single symptom and TCM syndrome score. Conclusion： It is feasible to set up a unified method to reduce blood pressure for a certain syndrome of hypertension. The patient＆#39;s concentration,the patient＆#39;s feed-back to the needle,and the reasonable selection of acupoints may play a decisive role in the acupuncture effect.
