IgA肾病患者的淋巴细胞亚群分布及与疾病的关系

Distribution patterns of lymphoid subgroups and IgA nephropathy:a correlative study

目的分析门急诊IgA肾病患者的淋巴亚群分布及与疾病的关系。方法回顾82例门急诊IgA肾病患者的临床资料,统计患者CD_4、CD_8、CD_4/CD_8水平,选取同期健康体检人群作为对照组;对比观察不同病理损伤和临床表现的IgA肾病患者淋巴细胞亚群分布情况。结果 IgA肾病患者的CD_4显著高于健康对照组(P<0.05),但两组间CD_8、CD_4/CD_8差异均无统计学意义(P>0.05);IgA肾病患者中有肾小球及间质硬化改变患者的CD_4/CD_8显著高于无改变患者和对照组(P<0.05),但无肾小球及间质硬化改变患者和对照组的CD_4/CD_8间差异无统计学意义(P>0.05)。IgA肾病患者中,有肾小球及间质硬化改变患者的24 h尿蛋白水平>1.0 g比例显著高于无肾小球及间质硬化改变患者(P<0.05)。结论不同病理损伤和临床表现的IgA肾病患者淋巴细胞亚群分布不同。

Objective To explore the correlation of the distribution patterns of lymphoid subgroups to IgA nephropathy. Method The clinical data of 82 patients of IgA nephropathy were firstly collected and then the levels of CD4, CD8 , CD4/ CD8 of the patients were documented. Next, healthy subjects were chosen as the control group. Lastly, the distribution patterns of lymphoid subgroups of the two groups were compared. Findings The CD4 of experimental group was significantly higher than the control group （ P 〈 0.05 ）. However, the differences in the levels of CDs , CD4/CD8 between the two groups were not significant （P 〉 0.05 ）. The level of CD4/CD8 of the patients in the experimental group with glomerular and interstitial sclerosis was significantly higher than that of those without glomerular and interstitial sclerosis and that of the control group （P 〈 0.05）. However, the difference in the level of CD4/CD8 between those in the experimental group without glomerular and interstitial sclerosis and the control group was not significant （ P 〉 0.05 ）. The 24h urinary protein level 〉 1.0g ratio of those in the experimental group with glomcrular and interstitial sclerosis was significantly higher than those without glomerular and interstitial sclerosis （ P 〈 0.05）. Conclusion The distribution patterns of lymphocyte subsets in patients with IgA nephropathy have different pathological and clinical implications.
