目的分析阿尔茨海默病(Alzheimer's disease,AD)患者的医疗负担情况,为阿尔茨海默病预防与控制提供参考依据。方法以问卷形式调查68例AD患者包括年龄、性别、婚姻、职业、文化程度等社会人口学资料;以及直接医疗负担,包括门诊费调查患者近3个月门诊就诊次数,近3个月最后一次门诊就诊费用,住院患者近12个月的住院次数,以及近12个月最后一次住院所致费用。结果过去3个月,曾有51例(75.00%)门诊就诊,平均门诊就诊次数2.96次,最后一次门诊就诊总费用平均为510.89±358.76元;不同性别和不同居住地最后一次门诊就诊总费用差异无统计学意义(P>0.05);但农村合作医疗患者因AD导致的药费比其他付费方式患者更低(P<0.05)。过去12个月,住院有44例AD患者,平均住院次数为4.14次(1~12),平均每次住院天数为26.93±14.37天;最后一次住院的总费用平均为11497.10±5234.33元;不同性别、不同居住地、不同付费方式患者间最后一次住院总费用差异无统计学意义(P>0.05)。结论阿尔茨海默病患者直接医疗负担较重,政府部门在配置卫生资源时应充分考虑其经济负担,改善AD患者的卫生保健现状。
Objective To analyze the medical burden of patients with Alzheimer's disease (AD) so as to provide intelligence for informed prevention and control of Alzheimer's disease. Method A questionnaire based survey was firstly conducted on 68 patients with AD to collect their demographic data such as age, sex, marriage, occupation, and educational level as well as the burden of direct medical costs imposed on them, including outpatient service, outpatient visits, and the last outpatient treatment costs in the recent 3 months. Hospitalization costs were investigated for the number of hospitalizations in the last 12 months and the cost of hospitalization for the last 12 months. Findings Over the past three months, there were 51 (75.00%) outpatient visits. The average number of outpatient visits in the past three months was 2.96. The total cost of the last outpatient treatment in the past three months averaged 510. 89 ± 358. 76 yuan. There was no significant difference in the total cost of the last outpatient service between different sexes and different places of residence. However, the cost of AD caused by AD in rural cooperative medical patients was lower than that of other patients ( P 〈 0.05 ). 44patients with AD were hospitalized in the past 12 months. The average number of hospitalizations was 4. 14 (1-12). The average number of hospitalizations per day was 26.93 ±14.37 days. The total cost of the last hospital stay in the past 12 months averaged 11497.10 ± 5234.33 yuan. There was no statistically significant difference in the total cost of the last hospitalization between the different sexes, the different places of residence, and different payment methods. Conclusion The direct medical burden of patients with Alzheimer's disease is heavier. Government departments should fully consider their economic burden and improve the health care status of AD patients when disposing health resources.
