经直肠超声联合前列腺特异性抗原检测诊断前列腺癌的价值

Clinical Value of Transrectal Ultrasound Combined with PSA in the Diagnosis of Prostate Cancer

目的探讨经直肠超声联合前列腺特异性抗原(PSA)检测在诊断前列腺癌中的应用价值。方法对医院收治的326例行经直肠超声及血清PSA水平检测提示可疑前列腺癌的临床资料进行回顾性分析。将手术、病理检查或者跟踪随访结果作为"金标准",评估经直肠超声、PSA检测、经直肠超声联合PSA检测在前列腺癌诊断中的灵敏度、特异度、准确度、阳性预测值和阴性预测值。结果 326例可疑前列腺癌患者中,共有23例经手术、病理检查或者跟踪随访确定患有前列腺癌,患病率为7.06%;三种检测方法中,经直肠超声联合PSA检测灵敏度最低,但是特异度、准确度、阳性预测值和阴性预测值均相对较高。结论经直肠超声检查联合PSA检测诊断前列腺癌诊断特异度和准确度较高,可减少漏诊和误诊情况。

Objective To investigate the value and significance of transrectal ultrasound combined with prostate spe- cific antigen （PSA） in the diagnosis of prostate cancer. Methods The clinical data of 326 patients with suspected prostate cancer who were diagnosed by transrectal ultrasound and serum PSA level in our hospital were retrospectively analyzed. All of the subjects were followed up to determine the positive or negative prostate cancer. The surgery, pathology or follow - up re- sults were recorded as the ＂gold standard＂, and the specificity, accuracy, positive predictive value and negative predictive value of transrectal ultrasound, transrectal ultrasound combined with PSA detection in the diagnosis of prostate cancer were e- valuated. Results Among 326 patients with suspected prostate cancer, 23 cases were diagnosed with prostate cancer by sur- gery, pathology or follow - up, with the prevalence rate of 7.06%. The sensitivity of transrectal ultrasound combined with PSA detection in the three kinds of detection methods was the lowest, but the specificity, accuracy, positive predictive value and negative predictive value were relatively higher. Conclusion Transreetal uhrasonography combined with PSA can im- prove the specificity and accuracy in the diagnosis of prostate cancer, which can reduce misdiagnosis and missed diagnosis.
