止痛化癥胶囊联合孕三烯酮胶囊治疗子宫腺肌病临床观察

Clinical Observation of Zhitong Huazheng Capsules Combined with Gestrinone Capsules for Adenomyosis

目的:观察止痛化癥胶囊联合孕三烯酮胶囊治疗子宫腺肌病的临床疗效,及对患者血液高凝状态的改善情况。方法:按照随机数字表法将86例子宫腺肌病患者分为观察组和对照组各43例。2组均给予孕三烯酮胶囊治疗,观察组另给予止痛化癥胶囊治疗。观察2组治疗前后的凝血指标[活化部分凝血活酶时间(APTT)、血浆凝血酶时间(TT)、血浆纤维蛋白原(Fi B)]、子宫体积及痛经评分的变化,比较2组临床疗效,随访1年后统计复发率。结果:治疗后,2组APTT、TT均较治疗前缩短(P<0.05),Fi B水平均较治疗前降低(P<0.05);观察组APTT、TT均短于对照组(P<0.05),Fi B水平低于对照组(P<0.05)。治疗后,2组子宫体积均较治疗前减小(P<0.05),痛经评分均较治疗前降低(P<0.05);观察组的子宫体积小于对照组(P<0.05),痛经评分低于对照组(P<0.05)。观察组总有效率88.37%,高于对照组的69.77%(P<0.05)。随访1年,观察组复发率7.89%,对照组复发率33.33%,2组比较,差异有统计学意义(P<0.05)。结论:止痛化癥胶囊联合孕三烯酮胶囊治疗子宫腺肌病,能有效改善血液高凝状态,减小子宫体积,减轻痛经状况,继而达到提高临床疗效和降低复发率的目的。

Objective：To observe the clinical effect of Zhitong Huazheng capsules combined with gestrinone capsules for adenomyosis and its effect on the improvement of high blood coagulation state. Methods：Divided 86 cases of patients with adenomyosis into the observation group and the control group randomly,43 cases in each group. Both groups were treated with gestrinone capsules,while the observation group was additionally treated with Zhitong Huazheng capsules. Observed changes of coagulation indexes [activated partial thromboplastin time（APTT）,plasma thrombin time（TT） and fibrinogen（Fib）],uterine volume and dysmenorrhea score in both groups before and after treatment. Compared the clinical effect in both groups and collected the recurrence rate after following up for one year. Results：After treatment,APTT and TT in both groups were shortened when compared with those before treatment（P〈0.05）,while Fib levels in both groups were reduced（P〈0.05）.AP T T and T T in the observation group were both shorter than those in the control group（P〈0.05）,and Fib levels in the observation group was less than those in the control group（P〈0.05）. After treatment,comparing with those before treatment,uterine volumes and dysmenorrhea scores in both groups were decreased（P〈0.05）. Uterine volumes in the observation group were smaller than those in the control group（P〈0.05）, and dysmenorrhea scores in the observation group were less than those in the control group（P〈0.05）. The total effective rate was 88.37% in the observation group,being higher than 69.77% in the control group（P〈0.05）. After one-year of follow up,the recurrence rate was 7.89% in the observation group and 33.33%in the control group, the difference being significant（P〈0.05）. Conclusion： The therapy of Zhitong Huazheng capsules combined with gestrinone capsules for adenomyosis can effectively improve high blood coagulation state, reduce uterine volume and relieve dysmenorrhea in order to achieve the purposes of enhancing clinical effect and bringing down the recurrence rate.
