MURU对子宫肌瘤患者子宫血流动力学和卵巢功能的影响

Effects of MURU on uterine hemodynamics and ovarian function in patients with uterine leiomyoma

目的:探讨肌壁大部切除-子宫重建术(MURU)对子宫肌瘤患者子宫血流动力学和卵巢功能的影响。方法:回顾性分析100例接受手术治疗的子宫肌瘤患者的临床资料,根据其手术方式分为子宫肌瘤剔除术组42例和MURU组58例。观察两组患者手术前后子宫血流动力学和子宫体积的变化,比较两组患者手术前后性生活质量和激素水平的差异。结果:两组患者术前子宫血流动力学和子宫体积差异无统计学意义,术后6个月收缩期最大血流速度(PSV)、舒张末期血流速度(EDV)、血管阻力指数(RI)和血管搏动指数(PI)差异无统计学意义,MURU组子宫体积较子宫肌瘤剔除术组小(P<0.001);两组患者术前和术后6个月性生活质量差异无统计学意义(P>0.05),雌二醇(E2)、卵泡刺激素(FSH)、抗穆勒管激素(AMH)和促黄体生成素(LH)水平差异无统计学意义(P>0.05)。结论:MURU对子宫肌瘤患者的子宫血流动力学、卵巢功能影响较小,具有临床应用价值。

Objective： To investigate the effect of major muscle wall resection and reconstruction（MURU） on uterine hemodynamics and ovarian function in patients with uterine fibroids.Methods： The clinical data of 100 patients with uterine fibroids treated surgically were retrospectively analyzed.According to the operation mode,they were divided into myomectomy group and subtotal-uterine muscle wall reconstruction group.The changes of uterine blood flow and uterine volume before and after operation in the two groups were observed,and the difference of sexual life quality and hormone levels between the two groups before and after operation was compared.Results： Six months after surgery, no difference of PSV, EDV, RI and PI was found between the two groups, while the uterine volume of patients in MURU group was smaller than that of those in uterus myomectomy group（P＆lt;0.001）.There was no significant difference in the quality of sexual life（P〉0.05） between the two groups before and 6 months after operation.E2, FSH, AMH and LH levels were not different between the two groups before and 6 months after surgery.Conclusion： The major part of the muscle wall resection and uterine reconstruction has little effect on the uterine hemodynamics and ovarian function in the patients with uterine fibroids, which is valuable in clinical application.
