联合检测PVW、SSM、LSM对肝硬化发生胃底静脉曲张出血风险的预测价值 被引量:4

摘要 目的评估门静脉宽度(PVW)、脾脏硬度测量(SSM)和肝脏硬度测量(LSM)对肝硬化发生胃底静脉曲张出血的预测价值。方法选取2014年9月至2016年1月因肝硬化就诊于本院消化内科的67例患者为研究对象,以胃镜检查结果分为重度组10例和非重度组57例。检测AST、ALT、PLT、PVW、SSM、LSM。结果重度组AST[(62.22±27.8)U/L]、ALT[(70.24±20.93)U/L]、PLT[(87.00±23.00)×109/L]、LSM[(32.71±16.04)kPa]与非重度组AST[(49.71±13.9)U/L]、ALT[(59.17±17.21)U/L]、PLT[(101.00±35.00)×109/L]、LSM[(24.39±13.66)kPa]差异无统计意义(P>0.05)。重度组与非重度组PVW[(1.29±0.16)对(1.15±0.12)cm]、SSM[(52.79±12.93)对(35.53±19.89)kPa]差异有统计学意义(P<0.05)。单一检测PVW、SSM、LSM的AUROC面积分别为0.6264、0.6533、0.5602,PVW+SSM、SSM+LSM和PVW+SSM+LSM的AUROC面积分别为0.7151、0.7393、0.8218,均高于单一检测效能。PVW+SSM+LSM联合检测的灵敏度和特异度提高到91.92%和83.72%。结论 PVW、SSM、LSM对肝硬化发生胃底静脉曲张均有一定的诊断价值,但三项指标联合诊断的灵敏度和特异度更高,对胃底静脉曲张出血风险有更高的预测价值。
